摘要
目的探讨一孔式巴德微创旋切术在单侧直径<2cm多发乳腺纤维瘤患者中的应用价值。方法回顾性分析2017年10月-2019年9月间收治的124例多发乳腺纤维瘤患者临床资料,所有患者均行手术治疗,根据手术方式将患者分为观察组(微创旋切术,n=52)和对照组(常规切除手术,n=72)。比较两组围术期情况(手术时间、瘢痕大小、术中出血量、住院时间),术后6个月评估两组患者临床疗效、两组瘢痕情况[温哥华瘢痕量表(VSS)]及患者满意度,记录术后6个月内并发症发生率。结果①观察组手术时间、瘢痕大小、术中出血量、住院时间均小于对照组(P<0.05);②术后6个月,两组总有效率对比差异无统计学意义(P>0.05),观察组显效率高于对照组(P<0.05);③观察组VSS得分低于对照组(P<0.05),观察组患者满意度得分高于对照组(P<0.05);④观察组总并发症发生率高于对照组(P<0.05)。结论一孔式巴德微创旋切术在小病灶多发纤维瘤患者中应用价值较高,在治疗效果、美观度和安全性均更高。
Objective To explore the application value of one-hole Bard minimally invasive rotary resection in patients with multiple breast fibroma with unilateral diameter<2cm.Methods The clinical data of 124 patients with multiple breast fibroma treated between October 2017 and September 2019 were retrospectively analyzed.All patients underwent surgical treatment,and the patients were divided into observation group(minimally invasive rotary resection,n=52)and control group(conventional resection,n=72)according to the surgical methods.The perioperative conditions(surgical time,intraoperative blood loss,scar size,hospital stay)were compared between the two groups,and the clinical efficacy,scar conditions(Vancouver Scar Scale,VSS)and patient satisfaction of the two groups were evaluated at 6 months after surgery,and the incidence rates of postoperative complications were recorded.Results①The surgical time,scar size,intraoperative blood loss and hospital stay in observation group were all less than those in control group(P<0.05).②There was no significant difference in the total effective rate between the two groups at 6 months after surgery(P>0.05),and the markedly effective rate in observation group was higher than that in control group(P<0.05).③The VSS score in observation group was lower than that in control group(P<0.05),and the patient satisfaction score in observation group was higher than that in control group(P<0.05).④The total incidence rate of complications in observation group was higher than that in control group(P<0.05).Conclusion One-hole Bard minimally invasive rotary resection has higher application value in patients with small lesions and multiple fibroma,and it has higher therapeutic effects,aesthetics and safety.
