以护士为主导的专科小组协同志愿者参与管理模式对社区糖尿病患者低血糖的影响

Effects of nurse-led specialist group and volunteer participation management model on hypoglycemia in community diabetic patients

目的研究以护士为主导的专科小组协同志愿者参与管理模式对社区糖尿病患者低血糖的影响。方法选取昆山市第四人民医院2018年1月至2020年6月收治的60例糖尿病患者, 数字表法随机分为对照组(30例)和观察组(30例)。对照组行常规护理, 观察组采用以护士为主导的专科小组协同志愿者参与管理模式。比较两组干预1、3、6个月后低血糖发生情况, 分析两组干预前后血糖波动情况〔血糖变异系数(GluCV)、血糖变化幅度(GluSD)、平均血糖浓度(GluAve)〕及自我效能评分, 比较两组护理满意度。结果观察组干预1个月后低血糖发生率为43.33%, 与对照组的46.67%比较差异无统计学意义(P>0.05);观察组干预3、6个月后低血糖发生率分别为6.67%、3.33%, 均低于对照组的26.67%、23.33%(P<0.05)。两组干预后GluCV、GluSD、GluAve较干预前降低(P<0.05);观察组干预后GluCV、GluSD、GluAve低于对照组(P<0.05)。两组干预后上述指标评分均较干预前升高(P<0.05);观察组干预后饮食控制、药物治疗、规律运动、血糖控制、足部护理与处理高低血糖评分及总分高于对照组(P<0.05)。观察组护理满意度为90.00%, 高于对照组的63.33%(P<0.05)。结论以护士为主导的专科小组协同志愿者参与管理模式用于社区糖尿病患者护理中, 可降低低血糖发生率, 提高自我管理能力, 值得临床推广。

Objective To study the effect of nurse-led specialist group and volunteer participation management mode on hypoglycemia in community diabetic patients.Methods A total of 60 diabetic patients admitted to the hospital from January 2018 to June 2020 were randomly divided into control group(30 cases)and observation group(30 cases)by number table method.The control group received routine nursing,and the observation group adopted nurse-led specialist group and volunteer participation management mode.The occurrence of hypoglycemia in the two groups after 1,3 and 6 months of intervention was compared.The blood glucose fluctuation〔coefficient of variation of blood sugar(GluCV),standard deviation of blood glucose(GluSD),average of blood glucose(GluAve)〕and self-efficacy score before and after intervention were analyzed,and the nursing satisfaction of the two groups was compared.Results The incidence of hypoglycemia in the observation group was 43.33%after 1 month of intervention,which was not statistically significant compared with 46.67%in the control group(P>0.05).The incidence of hypoglycemia in the observation group was 6.67%,3.33%respectively 3 and 6 months after intervention,which were lower than that in the control group 26.67%,23.33%(P<0.05).GluCV,GluSD and GluAve in the two groups after intervention were lower than those before intervention(P<0.05).After intervention,the observation group's diet control,drug therapy,regular exercise,blood glucose control,foot care and treatment of high and low blood glucose score and total score were higher than those of the control group(P<0.05).The nursing satisfaction of observation group was 90.00%,which was higher than that of control group 63.33%(P<0.05).Conclusions The nurse-led specialist group and volunteer participation management mode can reduce the incidence of hypoglycemia and improve self-management ability in the nursing of community diabetes patients,which is worthy of clinical promotion.
