指掌侧固有动脉背侧支逆行岛状皮瓣修复手指远端皮肤缺损的临床疗效及安全性分析

Clinical efficacy and safety of reverse island flap with dorsal branch of proper palmar digital artery for repairing skin defect of distal finger

目的:分析指掌侧固有动脉背侧支逆行岛状皮瓣修复手指远端皮肤缺损的临床疗效及安全性。方法:选取2017年2月~2020年6月住院治疗的50例手指远端皮肤缺损患者,以双盲随机抽样法分组(每组病例数共计25例),对照组采用腹部皮瓣修复,试验组采用指掌侧固有动脉背侧支逆行岛状皮瓣修复,对比两组临床指标、VAS评分、手功能评分、美容优良率、并发症发生率。结果:试验组手术时间与对照组比较,差异无统计学意义(P>0.05)。试验组住院时间短于对照组,治疗费用低于对照组,试验组术后2周术VAS评分低于对照组,手功能评分高于对照组,试验组美容优良率(96.00%)高于对照组(72.00%),试验组并发症发生率(4.00%)低于对照组(28.00%),差异均具有统计学意义(P<0.05)。结论:指掌侧固有动脉背侧支逆行岛状皮瓣修复可有效减轻手指远端皮肤缺损患者疼痛感,改善手功能,降低并发症发生率,且美容效果良好,值得借鉴。

Objective To analyze the clinical efficacy and safety of reverse island flap with dorsal branch of proper palmar digital artery for repairing skin defect of distal finger.Method 50 patients with distal finger skin defect in our hospital from February 2017 to June 2020 were selected and divided into two groups by double-blind random sampling method(25 cases in each group).The control group was treated with abdominal flap,and the experimental group was treated with reverse island flap of dorsal branch of proper digital artery.The clinical indexes,VAS score,hand function score,excellent and good rate of cosmetology and complication were compared between the two groups.Results There was no significant difference in operation time between the experimental group and the control group(P>0.05).The hospitalization time of the experimental group was shorter than that of the control group,the treatment cost was lower than that of the control group,the VAS score of the experimental group was lower than that of the control group,the hand function score was higher than that of the control group,the beauty excellent rate of the experimental group(96.00%)was higher than that of the control group(72.00%),the incidence of complications in the experimental group(4.00%)was lower than that of the control group(28.00%),the differences were statistically significant(P<0.05).Conclusion The reverse island flap with dorsal branch of proper palmar digital artery can effectively relieve the pain of patients with skin defect of distal finger,improve hand function,reduce the incidence of complications,and has good cosmetic effect,which is worthy of reference.
