3D-ASL技术对车祸所致创伤后应激障碍患者脑血流灌注的研究 被引量:1

摘要 目的:探讨创伤后应激障碍(PTSD)患者的感兴趣(ROI)相关脑区脑血流灌注变化。方法:收集车祸后患者28例,其中PTSD患者13例,非PTSD患者15例,正常健康志愿者15例,三组均行3.0T脑功能磁共振3D-ASL扫描,并对所有原始数据在GE ADW4.6工作站进行图像后处理,测得感兴趣(ROI)相关脑区的CBF值。结果:PTSD组较非PTSD、正常人胼胝体压部、右侧杏仁核CBF均增加;PTSD较非PTSD、正常人右侧颞叶CBF均减少;PTSD患者右侧海马、胼胝体膝部CBF较正常人减少,较非PTSD增加。结论:3D-ASL技术阐释了PTSD患者机体可能通过一系列神经生化机制导致右侧海马、右侧颞叶、右侧杏仁核脑血流灌注变化,右侧颞叶脑血流灌注改善尚且有助于PTSD患者病恢复。 Objective To investigate the changes of cerebral blood perfusion in ROI-related cerebral regions in patients with PTSD.Method A total of 28 patients(13 PTSD patients,15 non-PTSD patients,and 15 normal healthy volunteers)were collected after car accident.3.0T functional magnetic resonance 3D-ASL scanning was performed in all the three groups.All the original data were post-processed on GE ADW4.6 workstation,and CBF values in ROI-related brain regions were measured.Results The CBF of corpus callosum and right amygdala increased in PTSD group compared with non-PTSD and normal subjects.PTSD decreased compared with non-PTSD and normal subjects in right temporal lobe CBF.The CBF in the right hippocampus and knee of corpus callosum of PTSD patients decreased compared with normal controls,but increased compared with non-PTSD patients.Conclusion 3D-ASL technology explains that the body of PTSD patients may lead to the changes of cerebral blood flow perfusion in the right hippocampus,right temporal lobe and right amygdala through a series of neurobiochemical mechanisms,and the improvement of cerebral blood flow perfusion in the right temporal lobe is even helpful to the recovery of PTSD patients.
