小切口面部除皱术联合自体脂肪面部填充在面部年轻化手术中的应用及近远期疗效评价

Application of Small Incision Rhytidectomy Combined with Autologous Fat Facial Filling in Facial Rejuvenation Surgery and Evaluation of Short-term and Long-term Efficacy

目的探讨小切口面部除皱术联合自体脂肪面部填充在面部年轻化手术中的应用价值,并评估其近远期疗效。方法选取2017年6月—2018年5月本院收治的86例拟行年轻化手术治疗的患者,根据治疗方式分为对照组和研究组,对照组接受小切口面部除皱术治疗,研究组接受小切口面部除皱术联合自体脂肪面部填充治疗。测量2组手术前后瞳孔点至鼻唇沟与瞳孔垂线交点距离、双侧颧点间距及ABC弧度,统计术后并发症发生情况;比较2组近远期(术后、术后1年)临床疗效及满意度。结果术前2组瞳孔点至鼻唇沟与瞳孔垂线交点距离、双侧颧点间距及ABC弧度比较,差异无统计学意义(P<0.05);术后2组瞳孔点至鼻唇沟与瞳孔垂线交点距离、ABC弧度均较术前降低,且研究组低于对照组,差异有统计学意义(P<0.05);术后研究组双侧颧点间距较术前宽(P<0.05),而对照组手术前后双侧颧点间距差异无统计学意义(P>0.05)。2组术后钙化、感染、脂肪液化及纤维囊化发生率及总并发症发生率比较,差异均无统计学意义(P>0.05);2组术后近远期临床疗效等级分布比较,差异有统计学意义(P<0.05),且研究组近远期优良率高于对照组(P<0.05);研究组术后满意度及随访1年满意度均高于对照组,差异有统计学意义(P<0.05)。结论小切口面部除皱术联合自体脂肪面部填充可有效改善面部松弛状况,近远期疗效确切,安全可靠,患者满意度高。

Objective In order to explore the application value of small incision rhytidectomy combined with autologous fat facial filling in facial rejuvenation surgery,and to evaluate its short-term and long-term effects.Methods Eighty-six patients treated with light surgery were selected from June 2017 to may 2018,the patients were divided into control group and study group according to the treatment.The control group was treated by small incision rhytidectomy,and the study group was treated by small incision rhytidectomy combined with autogenous fat facial filling.Before and after operation,the distance from pupil point to the intersection of nasolabial groove and pupil vertical line,the distance between bilateral zygomatic points and ABC radian were measured.The postoperative complications were analyzed.The clinical efficacy and satisfaction of the two groups were compared.Results There were no significant differences in the distance between pupil point and nasolabial sulcus and pupil vertical intersection,the distance between zygomatic points and ABC radian between the two groups before operation(P<0.05).After operation,the distance from pupil point to nasolabial groove and pupil vertical intersection point and ABC radian of the two groups were lower than those of the control group(P<0.05).After operation,the distance between the zygomatic points in the study group was wider than that before operation(P<0.05),but there were no significant differences between the two zygomatic points in the control group before and after operation(P>0.05).There was no significant differences between the two groups in the incidence of calcification,infection,fat liquefaction,fibrocystic and total complications(P>0.05).There were significant differences between the two groups(P<0.05),and the excellent rate of the study group was higher than that of the control group(P<0.05).The postoperative satisfaction and one-year follow-up satisfaction of the study group were higher than those of the control group(P<0.05).Conclusion Small incision rhytidectomy combined with autologous fat facial filling can effectively improve facial relaxation.The short-term and long-term results are exact,safe and reliable,and patient satisfaction is high.
