肥胖者血清脂联素水平与代谢状况的关系

Relationship Between Serum Adiponectin and Metabolic Status in Obesity Individuals

目的调查肥胖者血清中常见细胞因子水平与其代谢状况的关系,初步探讨肥胖代谢异质性的分子机制。方法以浙江代谢综合征队列基线人群中代谢异常和代谢正常肥胖者为研究对象,采用Luminex方法检测其血清中脂联素、血管内皮生长因子A(VEGF-A)、干扰素-γ(IFN-γ)、胰岛素生长因子(IGF-1)、白细胞介素-6(IL-6)等细胞因子水平,比较各细胞因子水平在两组间的差异。结果代谢正常肥胖组血清脂联素水平的中位数为3.09μg/mL,四分位数间距(P;-P;)为2.00~4.48μg/mL,显著高于代谢异常肥胖组的2.31μg/mL(P_(25)-P_(75)为1.73~3.43μg/mL)(P=0.011)。其它细胞因子水平两组间差异无统计学意义(均P>0.05)。二元Logistic回归结果显示,高脂联素水平可以降低肥胖代谢异常的发生风险,OR值为0.25(95%CI:0.09~0.76,P=0.014);高脂联素水平也能降低高血压的发生风险,OR值为0.13(95%CI:0.04~0.45,P=0.001);与高三酰甘油血症也呈边缘性负相关,OR值为0.35(95%CI:0.12~1.04,P=0.059)。结论血清脂联素与肥胖的代谢状况有关,高脂联素水平会降低肥胖代谢异常风险。

Objective To investigate the relationship between levels of common cytokines in serum and metabolic status among obesity population,and to explore the molecular mechanism of obesity metabolic heterogeneity.Methods The subjects with metabolically abnormal obesity(MAO)and those with metabolically normal obesity(MNO)were enrolled from Zhejiang Metabolic syndrome cohort.The levels of cytokine such as adiponectin,endothelial growth factor A(VEGF-A),interferon-γ(IFN-γ),insulin growth factor(IGF-1),interleukin 6(IL-6),and so on were detected using Luminex method and the differences compared between MAO and MNO.Results The median serum adiponectin level in the MNO group was 3.09μg/mL,and the interquartile range(P;-P;)was 2.00-4.48,which was significantly higher than that of the MAO goup at 2.31μg/mL(P_(25)-P_(75)for 1.73-3.43)(P=0.011).The differences between the two groups in levels of other cytokines were not statistically significant(P>0.05).Binary logistic regression showed that higher level of adiponectin reduced the risk of MAO,with an OR value of 0.25(95%CI:0.09-0.76,P=0.014).Higher level of adiponectin also reduced the risk of hypertension with an OR of 0.13(95%CI:0.04-0.45,P=0.001)and also showed a borderline negative correlation with high triglycerides with an OR of 0.35(95%CI:0.12-1.04)(P=0.059).Conclusion Serum adiponectin is associated with the metabolic status of obesity,and high adiponectin levels reduce the risk of metabolic abnormality in obesity.
