云南德宏潞西地区傣族高血压患者ACEI类药物疗效观察

Observation on the efficacy of ACEI drugs for Dai hypertensive in Luxi area of Dehong prefecture of Yunnan Province

目的回顾性分析傣族与汉族高血压患者使用同一种ACEI药物的降压疗效,观察傣族应用ACEI类药物临床疗效敏感性,为探讨傣族种族特性对高血压降压疗效影响提供参考依据。方法收集2017年8月-2019年9月于德宏州人民医院心内科就诊的当地325例原发性高血压患者,匹配对照后分为傣族及汉族2组,观察其使用ACEI类药物治疗第2、4、8周的收缩压、舒张压、心率及药物使用情况,评估ACEI药物降压疗效及血压达标情况并进行比较。结果发现在血压控制达标的患者中,傣族组和汉族组ACEI药物使用剂量及达标时间差异均有统计学意义(P<0.05),但2组患者ACEI药物降压疗效及血压控制达标率均没有显著性差异(P>0.05)。结论同地域高血压患者中,傣族较汉族对ACEI的剂量需求小、血压控制达标时间短,但没有发现二者间ACEI药物降压疗效及达标率有差异,种族间基因多态性可能是个体对药物治疗反应差异的影响因素之一。

Objective To retrospectively analyze the antihypertensive efficacy of same ACEI drug use for the Dai and Han nationality hypertensive patients,and observe the sensitivity of clinical efficacy of ACEI drugs in the Dai nationality so as to provide reference for exploring the influence of the Dai ethnic characteristics on the antihypertensive efficacy of hypertension.Methods A total of 325 local patients with primary hypertension in treatment in the department of cardiology of Dehong prefecture people’s hospital from Aug. 2017 to Sep. 2019, and were divided into two groups of the Dai and the Han after matching and contrasting,observed their systolic blood pressure,diastolic blood pressure,heart rate and drug use at weeks 2,4 and 8 of treatment with ACEI drugs,assessed and compared the antihypertensive efficacy of ACEI drugs and blood pressure attainment in two groups.Results In the case of patients with standard blood pressure control,there was statistical significance in the differences of dose of ACEI drugs dosage and standard time in Dai and Han groups(P <0.05), but there was no statistical significance in the difference of antihypertensive efficacy of ACEI drugs and standard rate of blood pressure control between the two groups(P>0.05).Conclusions We found that among hypertensive patients in same geographical area, the dose requirement of ACEI was lower and the time to achieve blood pressure control was shorter among the Dai which compared with the Han,but there was no difference in the efficacy of ACEI drugs in reducing blood pressure and achieving the standard rate between the two groups.Gene polymorphisms between races may be one of the influencing factors for differences in individual response to drug therapy.
