摘要
目的 探讨不同生化指标对2型糖尿病患者血清癌胚抗原(CEA)、糖蛋白抗原199(CA19-9)和甲胎蛋白(AFP)水平的影响。方法 收集2018年1月—2020年6月在本院内分泌科住院的200例患者,检测血清CEA、CA19-9和AFP水平,分析不同生化指标与其相关性,并对数据进行统计学分析。结果 相关分析显示患者糖化血红蛋白(HbA1c)水平与CEA正相关,尿肌酐水平与CA19-9负相关,总胆固醇(TC)和低密度脂蛋白(LDLC)水平与AFP正相关;CEA在HbA1c未达标组显著高于HbA1c达标组,2组间差异有统计学意义(P<0.05);CA19-9在中、重度肾损伤患者中显著高于正常或轻度肾损伤,2组间差异有统计学意义(P<0.05);CEA在血脂达标与否2组间差异无统计学意义(P>0.05),但随着TC和LDLC四分位数存在升高趋势。结论 生化指标可影响2型糖尿病患者CEA、CA19-9、AFP水平;在血糖控制不佳时CEA和肾损伤患者CA19-9诊断特异性降低;血脂与CEA相关,但其对CEA影响仍需进一步研究。
Objective To investigate the effects of different biochemical indicators on the levels of serum carcinoembryonic antigen(CEA),glycoprotein antigen 199(CA19-9)and alpha-fetoprotein(AFP)in patients with type 2 diabetes.Methods A total of 120 patients were collected from January 2018 to June 2020 in our hospital.The levels of serum of CEA,CA19-9 and AFP were detected;different biochemical indicators and their correlation were analyzed,and then analyzed statistically.Results Correlation analysis showed that the level of glycosylated hemoglobin(HbA1c)was correlated with CEA,the level of urine creatinine was correlated with CA19-9,and the levels of total cholesterol(TC),high density lipoprotein(HDLC)and low density lipoprotein(LDLC)were correlated with AFP related;CEA was significantly higher in the HbA1c non-standard group than the standard group,and the difference between two groups was significantly(P<0.05);CA19-9 was significantly higher in patients with moderate and severe renal injury than normal or mild renal injury.The difference between two groups was statistically significantly(P<0.05);the difference of CEA was not statistically significant between the blood lipids non-standard and standard groups(P>0.05),but there was an increasing trend with TC and LDLC quartile.Conclusion Biochemical indicators may affect the levels of CEA,CA19-9,and AFP in patients with type 2 diabetes.The specificity of diagnosis for CEA in patients with poor blood glucose control and CA19-9 in renal injury might decrease.Blood lipid is related to CEA,but its impact still needs further studies.
