摘要
目的 了解河南省某医院结核病区结核分枝杆菌的耐药情况,为结核病的感染防控提供理论依据。方法 收集2017年-2020年本院收治的细菌培养阳性结核病患者,对人口特征及药敏结果进行统计分析。结果 共收集2 706例细菌培养阳性的结核病患者,其中MDR和XDR患者共312例;不同年份SDR、MDR、XDR耐药率差异均有统计学意义(P<0. 05),单R的耐药率有所提高,最高至5. 34%;MDR和PDR的耐药率下降至4. 82%和0. 14%。耐多药患者中初治患者最多,为145例;15种耐药谱检出前3位依次为H+R+E+S+O+K(61例)、H+R+S(53例)、H+R+S+O+K(40例)。结论 MDR和PDR耐药率有下降趋势,以初治患者最多,也多见于初治失败和复发者,加强耐药结核病患者的规范治疗和管理,遏制结核分枝杆菌的产生。
Objective To investigate the drug resistance of Mycobacterium tuberculosis in a hospital in Henan Province,so as to provide theoretical basis for the prevention and control of tuberculosis infection.Methods A statistical analysis was conducted on the population characteristics and drug sensitivity of bacterial culture-positive tuberculosis patients admitted to our hospital from 2017 to 2020.Results A total of 2706 tuberculosis patients were collected,including 312 cases of MDR and XDR.The drug resistance rates of SDR,MDR and XDR in different years had statistical significance(P<0.05).The drug resistance rate of single R increased to 5.34%.The resistance rates of MDR and PDR decreased to 4.82%and 0.14%.Among the multi drug resistant patients,145 patients were newly treated H+R+E+S+O+K(61 cases),H+R+S(53 cases)and H+R+S+04-K(40 cases)were the top three drug resistance profiles detected.Conclusion The drug resistance rate of MDR and PDR is decreasing,First-time patients are the most common,but they are also most common in patients who have failed first-time treatment and relapsed,the standard treatment and management of drug-resistant tuberculosis patients should be strengthened to control the production of Mycobacterium tuberculosis.
