胸腰椎骨折与肾损伤的相关性及早期尿隐血检测的临床意义

Correlation between Thoracolumbar Fracture and Renal Injury and Clinical Significance of Early Detection of Occult Blood in Urine

目的分析胸腰椎骨折与肾损伤的相关性及早期尿隐血检测的临床意义。方法选取从2018年5月至2019年5月收治于我院的100例围胸腰椎区损伤患者作为研究对象,对其进行影像学检查与早期尿常规研究,尿隐血阳性患者进行尿沉渣显微镜镜检。观察有效患者病历的受伤因素、尿隐血检测结果,给予尿隐血阳性患者加以评分,探索血尿阳性率和致伤因素、机体损伤严重程度和骨折分布之间的关系。结果交通事故致伤概率大于其他性质致伤概率,且暴力伤合并血尿阳性率大于其他性质损伤,P<0.05差异有统计学意义;将胸腰椎骨折分为胸腰段骨折T_(11)-L_(2)以及非胸腰段骨折,两种情况进行对比,胸腰段骨折合并血尿阳性率小于非胸腰段骨折概率,P<0.05差异有统计学意义;轻重损伤率显著大于其他损伤程度,胸腰椎骨折合并尿隐血阳性者主要表现为轻度损伤,P<0.05差异有统计学意义。结论胸腰椎骨折与肾损伤之间存在一定关联,需要对患者进行早期尿隐血检测,提高临床治疗效率。

Objective To analyze the correlation between thoracolumbar fracture and kidney injury and the clinical significance of early detection of occult blood in urine.Methods A total of 100 patients with thoracolumbar injury admitted to our hospital from May 2018 to May 2019 were selected as the research objects.Imaging examination and early urine routine study were performed on them,and urine sediment microscopy was performed on patients with positive occult blood.The injury factors and urine occult blood test results of valid patients'medical records were observed,and the patients with positive urine occult blood were scored to explore the relationship between the positive rate of hematuria and injury factors,the severity of body injury and fracture distribution.Results The probability of injury caused by traffic accident was higher than that caused by other injuries,and the positive rate of hematuria caused by violence was higher than that caused by other injuries,the difference was statistically significant(P<0.05).Thoracolumbar fractures were divided into thoracolumbar fractures T_(11)-L_(2) and nonthoracolumbar fractures.The positive rate of thoracolumbar fractures combined with hematuria was lower than the probability of non-thoracolumbar fractures(P<0.05),and the difference was statistically significant.The severity injury rate was significantly higher than other injuries,and the patients with thoracolumbar fracture combined with positive urine occultation were mainly mild injuries,P<0.05,the difference was statistically significant.Conclusion There is a certain correlation between thoracolumbar fracture and kidney injury.Early detection of occult blood in urine is necessary to improve the clinical treatment efficiency.
