摘要
目的探讨桃红四物汤加味在踝关节骨折围手术期中的临床应用效果。方法74例踝关节骨折患者作为研究对象,根据随机数字表法分为对照组和观察组,每组37例。对照组采用切开复位内固定手术治疗,观察组采用切开复位内固定手术联合桃红四物汤加味治疗。比较两组治疗前后的症候积分,血液流变学指标[全血高切粘度(WBHSV)、全血低切粘度(WBLSV)、血浆粘度(PSV)、红细胞比容(HCT)、血小板聚集率(PAgT)]水平及并发症发生率。结果治疗后14 d,两组的疼痛、肿胀、瘀斑、局部压痛、口渴尿赤症候积分均低于本组治疗前,且观察组的疼痛、肿胀、瘀斑、局部压痛、口渴尿赤症候积分分别为(0.72±0.24)、(0.86±0.26)、(0.61±0.21)、(0.79±0.25)、(0.67±0.24)分,均低于对照组的(1.79±0.45)、(1.57±0.35)、(1.64±0.39)、(1.59±0.38)、(1.66±0.42)分,差异具有统计学意义(P<0.05)。治疗后14 d,两组的WBHSV、WBLSV、PSV、HCT、PAgT水平均低于本组治疗前,且观察组的WBHSV、WBLSV、PSV、HCT、PAgT水平分别为(5.02±0.47)mPa·s、(7.23±0.46)mPa·s、(1.63±0.24)mPa·s、(40.16±5.31)%、(35.39±4.59)%,均低于对照组的(6.74±0.53)mPa·s、(8.39±0.53)mPa·s、(1.89±0.32)mPa·s、(48.95±5.98)%、(61.35±5.64)%,差异具有统计学意义(P<0.05)。观察组的并发症发生率5.41%低于对照组的21.62%,差异具有统计学意义(P<0.05)。结论桃红四物汤加味在踝关节骨折围手术期中应用有助于降低症候积分,改善患者血液流变学水平,降低并发症发生率,值得临床推广应用。
Objective To discuss the clinical application effect of modified Taohong Siwu Decoction in perioperative period of ankle fracture.Methods A total of 74 patients with ankle fracture as the research subjects were divided into control group and observation group according to random numerical table,with 37 cases in each group.The control group was treated with open reduction internal fixation,and the observation group was treated with open reduction internal fixation in combination with Taohong Siwu Decoction.Both groups were compared in terms of syndrome scores,hemorheology indicators[whole blood high-shear viscosity(WBHSV),whole blood low-shear viscosity(WBLSV),plasma viscosity(PSV),hematocrit(HCT),platelet aggregation rate(PAgT)]before and after treatment,and complication rate.Results 14 d after treatment,the scores of pain,swelling,ecchymosis,local tenderness,and thirst and urine redness in both groups were lower than those of this group before treatment;the scores of pain,swelling,ecchymosis,local tenderness,thirst and red urine of the observation group were(0.72±0.24),(0.86±0.26),(0.61±0.21),(0.79±0.25),(0.67±0.24)points,which were all lower than(1.79±0.45),(1.57±0.35),(1.64±0.39),(1.59±0.38),(1.66±0.42)points of the control group;all the differences were statistically significant(P<0.05).14 d after treatment,the levels of WBHSV,WBLSV,PSV,HCT,and PAgT in the two groups were lower than those before treatment in this group;the levels of WBHSV,WBLSV,PSV,HCT,and PAgT in the observation group were(5.02±0.47)mPa·s,(7.23±0.46)mPa·s,(1.63±0.24)mPa·s,(40.16±5.31)%,(35.39±4.59)%,which were all lower than(6.74±0.53)mPa·s,(8.39±0.53)mPa·s,(1.89±0.32)mPa·s,(48.95±5.98)%,(61.35±5.64)%in the control group;all the differences were statistically significant(P<0.05).The complication rate 5.41%of the observation group was lower than 21.62%of the control group,and the difference was statistically significant(P<0.05).Conclusion The application of modified Taohong Siwu Decoction in the perioperative period of ankle fracture can help reduce the symptom score,improve the patient’s hemorheology level,and reduce the complication rates,which is worthy of clinical promotion and application.
