基于D-S证据理论的区域医疗服务水平测度模型研究

Researches on the Regional Medical Service Level Measurement Model Based on D-S Evidence Theory

目的:从政府管理和患者感知视角,建立医疗服务可提供性、医疗服务可获得性、经济可负担性和体系有效性4个维度的区域医疗服务水平测度指标体系,构建区域医疗服务水平测度模型。方法:利用模糊粗糙集方法对医疗服务水平测度指标进行筛选,得到区域医疗服务水平测度指标体系。运用D-S证据理论合成AHP法确定的主观权重和熵值法确定的客观权重,得到指标的组合权重。将灰色关联分析法和D-S证据理论相结合,建立信息不对称环境下的区域医疗服务水平测度模型。结果:以江苏省13个地级市为例进行实证研究,医疗服务水平居于全省首位的是泰州市,医疗服务水平最低的地区是徐州市。结论:区域医疗服务水平测度模型能准确识别出医疗服务水平较好的地区,从不同维度比较和分析各地区的医疗服务水平情况,为各地区制定合理配置医疗资源相关政策提供理论依据。

Objective To establish a regional medical service level measurement index system with four dimensions of medical service availability,medical service availability,economic affordability and system effectiveness,and construct a regional medical service level measurement model from the perspective of government management and patients’perception.Methods Medical service level measurement indicators were screened with fuzzy rough set method to obtain the regional medical service level measurement indicator system.The subjective weights determined by AHP method and objective weights determined by entropy method were used to obtain its combination weights,based on D-S evidence theory.And combining the gray correlation analysis method with D-S evidence theory,a regional medical service level measurement model under asymmetric information environment was established.Results 13 cities in Jiangsu Province were taken as an example,in which the provincial highest level of medical services was in Taizhou City,while the lowest was in Xuzhou City.Conclusion The regional medical service level measurement model can accurately identify the regions with better medical service,and comparatively analyze the medical service levels of various regions from different dimensions,which can provide a theoretical basis for each region to formulate policies related to the rational allocation of medical resources.
