祛瘀生新方联合益生菌治疗溃疡性结肠炎的疗效分析

Analysis of Curative Effect on Quyu Shengxin Formula Combined with Probiotics in the Treatment of Ulcerative Colitis

【目的】探讨祛瘀生新方联合益生菌治疗溃疡性结肠炎(UC)脾胃气虚型患者的临床疗效。【方法】将164例溃疡性结肠炎脾胃气虚型患者随机分为研究组和对照组,每组各82例。对照组给予益生菌治疗,研究组在对照组基础上加用祛瘀生新方治疗,疗程为4周。观察2组患者治疗前后中医证候评分、血清炎性因子[肿瘤坏死因子α(TNF-α)、白细胞介素8(IL-8)、白细胞介素1β(IL-1β)]、免疫功能指标[血清T淋巴细胞亚群CD3^(+)、CD4^(+)、CD8^(+)水平及CD4^(+)/CD8^(+)比值]的变化情况,并评价2组的疗效。【结果】(1)研究期间,有5例患者脱落,最终共有159例患者完成试验,其中对照组为79例,研究组为80例。(2)治疗4周后,研究组的总有效率为92.50%(74/80),对照组为72.15%(57/79);组间比较,研究组的总有效率和总体疗效均明显优于对照组(P<0.01)。(3)治疗后,2组患者的腹痛、腹泻、黏液血便、食少纳差等中医证候评分及血清TNF-α、IL-8、IL-1β水平均较治疗前明显降低(P<0.05),且研究组的降低作用均明显优于对照组(P<0.01)。(4)治疗后,2组患者血清T淋巴细胞亚群CD3^(+)、CD4^(+)水平和CD4^(+)/CD8^(+)比值均较治疗前明显升高(P<0.05),CD8^(+)水平均较治疗前明显降低(P<0.05),且研究组对血清T淋巴细胞亚群CD3^(+)、CD4^(+)水平和CD4^(+)/CD8^(+)比值的升高作用及对CD8^(+)水平的降低作用均明显优于对照组(P<0.01)。【结论】祛瘀生新方联合益生菌治疗溃疡性结肠炎疗效确切,可显著减轻患者临床症状,有效缓解患者炎性反应和改善患者免疫功能,其疗效优于单用益生菌治疗。

Objective To investigate the clinical efficacy of Quyu Shengxin Formula combined with probiotics in the treatment of ulcerative colitis(UC). Methods A total of 164 patients of UC with spleen and stomach qi deficiency syndrome were randomly divided into study group and control group,82 cases in each group. The two groups were treated with probiotics,and additionally the study group was treated with Quyu Shengxin Formula for removing stasis and promoting regeneration,the course of treatment lasting 4 weeks. The changes of traditional Chinese medicine(TCM)syndrome scores,serum inflammatory factors of tumor necrosis factor alpha(TNF-α),interleukin 8(IL-8)and interleukin 1β(IL-1β),and immune function indexes of serum T lymphocyte subsets CD3^(+),CD4^(+),CD8^(+)levels and CD4^(+)/CD8^(+)ratio in the two groups were observed before and after treatment. After treatment,the clinical efficacy of the two groups was evaluated. Results(1)During the trial,5 patients fell off,and a total of 159 patients eventually completed the trial,including 79 patients in the control group and 80 patients in the study group.(2)After 4 weeks of treatment,the total effective rate in the study group was 92.50%(74/80)and was 72.15%(57/79)in the control group. The intergroup comparison showed that the total effective rate and overall curative effect of the study group were significantly superior to those of the control group(P < 0.01).(3)After treatment, the scores of TCM syndrome such as abdominal pain, diarrhea, mucous and bloody stool, poor appetite and the levels of serum TNF-α,IL-8 and IL-1β in the two groups were significantly decreased compared with those before treatment(P < 0.05),and the effect on decreasing the above indexes in the study group was significantly superior to that in the control group(P < 0.01).(4)After treatment,the levels of serum T lymphocyte subsets CD3^(+),CD4^(+)and the ratio of CD4^(+)/CD8^(+)in both groups were significantly increased compared with those before treatment(P < 0.05),and the level of CD8^(+)was significantly decreased compared with that before treatment(P < 0.05). The effect on increasing the levels of serum T lymphocyte subsets CD3^(+),CD4^(+)and the ratio of CD4^(+)/CD8^(+)and the effect on decreasing the level of CD8^(+)in the study group were significantly superior to that in the control group(P < 0.01). Conclusion Quyu Shengxin Formula combined with probiotics exerts certain effect in the treatment of ulcerative colitis through significantly relieving the clinical symptoms, effectively alleviating inflammatory response and improving immune function,and its efficacy is superior to that of the probiotics alone.
