“通因通用”法治疗小儿大便失禁应用体会

Experience in the Treatment of Children with Incontinence of Feces by the Method of“Treating Diarrhea with Purgative”

"通因通用"指用通利方药或通利功效的措施来治疗通泄症状的治法。对于儿童大便失禁,无论辨证为实证为主或虚实夹杂,只要有内结这一关键病机,均属于"通因通用"之下法范畴。治疗实证内结为主要表现之大便失禁患儿,宜通腑泻浊,方以大承气汤加减;治疗虚实夹杂之大便失禁患儿,以健脾补气、通腑泻浊为法,方以四君子汤合小承气汤加减。

“Treating diarrhea with purgative”refers to the use of purgative prescriptions or purgative therapies for the treatment of the syndrome of excess-type diarrhea.For children with incontinence of feces,the existence of internal retention in TCM pathogenesis is the very indication for the purgative therapy of“treating diarrhea with purgative”,regardless of the existence of predominant excess syndrome or deficiency interweaved with excess syndrome.For the treatment of children with incontinence of feces due to excess-type of internal retention,therapy of unblocking bowels and purging turbidity can be adopted,and the modified Da Chengqi Decoction can be chosen.For the treatment of children with incontinence of feces due to deficiency interweaved with excess syndrome,therapy of invigorating spleen and replenishing qi,unblocking bowels and purging turbidity can be adopted,and the modified Si Junzi Decoction combined with Xiao Chengqi Decoction can be chosen.
