董氏针刺配合“姐妹穴”埋线干预痰湿质胰岛素抵抗型多囊卵巢综合征临床研究

Clinical Study on Dong’s Acupuncture at “Sister Points” Thread Embedding Intervention of Phlegm-Dampness Insulin-Resistant Polycystic Ovary Syndrome

目的观察董氏针刺配穴"姐妹穴"埋线干预痰湿质胰岛素抵抗型多囊卵巢综合征(PCOS-IR)的临床疗效。方法将91例患者随机分为西药组(30例)、董针组(30例)和董针+埋线组(31例),西药组口服二甲双胍和优思明,董针组予董氏奇穴针刺(取穴妇科、还巢、天皇、人皇、姐妹一、二、三穴),董针+埋线组将姐妹一、二、三穴改为埋线,其它穴位针刺同董针组,3组均干预3个月经周期。观察3组治疗前后中医症状积分、月经平均周期、性激素水平[卵泡刺激素(FSH)、黄体生成素(LH)、睾酮(T)、LH/FSH]和胰岛素抵抗相关指标[空腹血糖(GLU)、空腹胰岛素(INS)、胰岛素抵抗指数(HOMA-IR)]的变化情况。结果 3组治疗后中医症状积分、LH、LH/FSH、T、GLU、INS和HOMA-IR均低于治疗前,差异均有统计学意义(P<0.01);董针组和董针+埋线组在降低LH/FSH、T、INS和HOMA-IR水平方面与西药组疗效相当(P>0.05);董针+埋线组在改善患者LH、GLU及中医症状积分和治疗后3个月月经周期方面的疗效优于西药组和单纯董针组(P<0.05),且董针组在改善患者中医症状积分、治疗后3个月月经周期方面优于西药组(P<0.05)。结论董氏针刺可以有效改善痰湿质PCOS-IR患者的性激素水平、胰岛素抵抗和月经情况,董氏针刺配合"姐妹穴"埋线的临床疗效更好。

Objective:To observe the clinical effect of Dong’s acupuncture at "sister points" thread embedding intervention of phlegm-dampness insulin-resistant polycystic ovary syndrome(PCOS-IR).Methods:91 patients were randomly divided into western medicine group(30 cases),Dong’s acupuncture group(30 cases) and Dong’s acupuncture + thread embedding group(31 cases).The western medicine group was given oral metformin and eusmin,and Dong’s acupuncture group was given Dong’s acupuncture at odd acupoints(selecting acupoints for gynecology,Huancao,Tianhuang,Renhuang,Sisters one,two,and three appoints).Dong’s acupuncture + thread embedding group changed sisters one,two and three appoints into thread embedding,and other acupuncture acupoints were the same in Dong’s acupuncture group.The three groups were all intervened for 3 menstrual cycles.The TCM symptom scores before and after treatment,average menstrual cycle,sex hormone levels [follicle stimulating hormone(FSH),luteinizing hormone(LH),testosterone(T),LH/FSH] and insulin resistance related indicators changes [fasting blood glucose(GLU),fasting insulin(INS),insulin resistance index(HOMA-IR)] of the three groups were observed.Results:The TCM symptom scores,LH,LH/FSH,T,GLU,INS and HOMA-IR after treatment in the three groups were all lower than those before treatment,and the differences were statistically significant(P<0.01).Dong’s acupuncture group and Dong’s acupuncture+ thread embedding group had the same effect as the western medicine group in reducing the levels of LH/FSH,T,INS and HOMA-IR(P>0.05).Dong’s acupunctue + thread embedding group could improve the LH,GLU and TCM symptom scores of the patients after treatment.The curative effect of menstrual cycle was better than that of the western medicine group and pure Dong’s acupuncture group(P<0.05),and Dong’s acupuncture group was better than western medicine group in improving the patient’s TCM symptom score and menstrual cycle after 3-month treatment(P<0.05).Conclusion:Dong’s acupuncture can effectively improve the level of sex hormones,insulin resistance and menstruation in PCOS-IR patients with phlegm and dampness and Dong’s acupuncture combined with "sister acupoints" has a better clinical effect.
