血清铁蛋白在肝病患者中的应用价值

Application Value of Serum Ferritin in Patients with Liver Disease

目的检测不同肝病患者血清铁蛋白(SF)水平,并分析血清SF水平在不同类型肝病患者中的应用价值。方法回顾性分析2020年1月—2021年3月该院收治的43例慢性肝炎患者的临床资料,将其纳入慢性肝炎组;收集同期该院收治的47例肝硬化患者的临床资料,将其纳入肝硬化组;另收集同期该院收治的41例肝癌患者的临床资料,将其纳入肝癌组;收集同期于该院体检显示无任何异常的41名健康体检者作为健康对照组。测定4组患者血清SF并分析血清SF水平在不同肝病患者中的应用价值。结果肝癌组血清SF水平最高(519.46±25.71)ng/mL,其次为肝硬化组(458.77±25.49)ng/mL,慢性肝炎组(396.14±25.38)ng/mL,健康对照组最低(102.79±23.11)ng/mL,4组间比较,差异有统计学意义(P<0.05);绘制ROC曲线,血清SF预测慢性肝炎、肝硬化及肝癌的AUC>0.70,有一定的预测价值。结论血清SF在不同类型肝病患者中,表达存在明显差异,在肝癌患者中的表达最高,临床上可通过检测血清SF水平,来辅助诊断肝病类型,为临床肝病早期诊疗提供依据。

Objective To detect serum ferritin(SF)levels in patients with different liver diseases,and to analyze the application value of serum SF levels in patients with different types of liver diseases.Methods For retrospective analysis,the clinical data of 43 patients with chronic hepatitis admitted to the hospital from January 2020 to March 2021 were collected and included in the chronic hepatitis group.The clinical data of 47 patients with liver cirrhosis admitted to this hospital during the same period were collected and included in the liver cirrhosis group.In addition,the clinical data of 41 liver cancer patients admitted to this hospital during the same period were collected and included in the liver cancer group.Collected 41 healthy subjects who showed no abnormalities during physical examination in the hospital during the same period as healthy control group.Measured the serum SF of 4 groups of patients and analyzed the application value of serum SF levels in patients with different liver diseases.Results The serum SF level in the liver cancer group was the highest(519.46±25.71)ng/mL,followed by the liver cirrhosis group(458.77±25.49)ng/mL,the chronic hepatitis group was(396.14±25.38)ng/mL,and the healthy control group was the lowest(102.79±23.11)ng/mL,the difference between the 4 groups was statistically significant(P<0.05).Draw ROC curve,serum SF predicts AUC of chronic hepatitis,liver cirrhosis and liver cancer>0.70,which had certain predictive value.Conclusion There are significant differences in the expression of serum SF in patients with different types of liver diseases,and the expression is highest in patients with liver cancer.Clinically,the level of serum SF can be detected to assist in the diagnosis of liver disease types in order to provide a basis for early clinical diagnosis and treatment of liver diseases.
