摘要
目的分析重症糖尿病足伤口护理中精细化护理的应用及控制血糖水平效果。方法选取2019年5月—2021年5月该院收治的重症糖尿病足伤患者74例,按照"护理差异"分为两组,各37例。对照组:常规护理;研究组:精细化护理,比较两组护理效果。结果两组护理前空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbAlc)组间对比,差异无统计学意义(P>0.05),护理后两组FPG、2 hPG、HbAlc均低于护理前,差异有统计学意义(P<0.05),组间对比研究组护理后FPG、2 hPG、HbAlc [(5.85±1.16)mmol/L、(7.82±0.81)mmol/L、(6.77±0.92)%]均低于对照组[(7.81±1.02)mmol/L、(9.49±0.79)mmol/L、(8.04±0.87)%],差异有统计学意义(t=7.718、8.978、6.101,P<0.05);两组护理前焦虑自评量表(SAS)、抑郁自评量表(SDS)评分组间对比,差异无统计学意义(P>0.05),护理后两组SAS、SDS评分均低于护理前,差异有统计学意义(P<0.05),组间对比研究组SAS、SDS评分[(45.32±4.12)分、(48.02±3.26)分]均低于对照组[(50.17±4.45)分、(51.79±3.35)分],差异有统计学意义(t=4.865、4.906,P<0.05);研究组伤口护理总有效率为94.59%,高于对照组78.38%,差异有统计学意义(χ^(2)=4.163,P<0.05);研究组护理满意度为97.30%,高于对照组81.08%,差异有统计学意义(χ^(2)=5.046,P<0.05)。结论在重症糖尿病足伤口护理中,采取精细化护理能够促进患者伤口愈合,同时合理控制血糖水平,应用价值更高。
Objective To analyze the application of refined care in the care of severe diabetic foot wounds and the effect of controlling blood glucose levels. Methods From May 2019 to May 2021, 74 patients with severe diabetic foot injuries in the hospital were selected. According to the "nursing difference", they were divided into two groups, each with 37 cases. Control group: routine care, study group: refined care. Compared the nursing effects of the two groups. Results There was no statistically significant difference between the two groups of fasting blood glucose(FPG) before nursing and 2 h postprandial blood glucose(2 hPG), glycosylated hemoglobin(HbAlc) between the two groups(P>0.05). FPG, 2 hPG and HbAlc in the two groups after nursing were lower than those before nursing, the difference was statistically significant(P<0.05). FPG, 2 hPG,HbAlc of the sutdy group [(5.85±1.16) mmol/L,(7.82±0.81) mmol/L,(6.77±0.92)%] after nursing were lower than the control group [(7.81 ±1.02) mmol/L,(9.49 ±0.79) mmol/L,(8.04 ±0.87)% ], the difference was statistically significant(t =7.718,8.978,6.101,P<0.05). The pre-care anxiety self-rating scale(SAS) and depression self-rating scale(SDS) scores were compared between the two groups, before nursing, and the difference was not statistically significant(P>0.05). The scores of SAS and SDS in the two groups after nursing were lower than those before nursing, the difference was statistically significant(P<0.05). The SAS and SDS scores of the study group [(45.32±4.12) points,(48.02±3.26) points] were lower than those of the control group [(50.17±4.45)points,(51.79±3.35) poin ts], the difference was statistically significant (t =4.865, 4.906, P <0.05). The total effective rate of wou nd care in the study group was 94.59%, which was higher than78.38% in the control group, the difference was statistically significant(χ^(2)=4.163,P<0.05). The nursing satisfaction degree of the study group was 97.30%, which was higher than 81.08% of the control group, the difference was statistically significant(χ^(2)=5.046,P<0.05). Conclusion In the care of severe diabetic foot wounds, the use of refined care can promote the healing of patients’ wounds. At the same time, the blood glucose level is controlled reasonably, and the application value is higher.
