摘要
目的分析个体化饮食护理对肺结核并糖尿病患者血糖水平的影响。方法选取2018年2月—2020年8月于该院接受治疗的84例肺结核并糖尿病患者进行探讨,按照时间顺序将患者分成两组,接受常规干预者归属于对照组,联合个体化饮食护理者归属于观察组,对比两组血糖及波动情况、生活质量。结果观察组空腹血糖(6.87±0.48)mmol/L,餐后2 h血糖(8.34±0.37)mmol/L,糖化血红蛋白(6.33±0.26)%,低于对照组,差异有统计学意义(t=9.149、16.164、14.044,P<0.05);观察组出院3个月的24 h血糖波动值为(2.03±0.24)mmol/L,空腹血糖变异系数(14.27±3.98)%,低于对照组,差异有统计学意义(t=23.331、9.712,P<0.05);观察组躯体质量、心理健康、角色以及社会功能评分分别是(76.74±3.23)分、(72.08±3.27)分、(78.18±3.58)分、(75.26±3.66)分,高于对照组,差异有统计学意义(t=8.306、6.909、7.641、7.055,P<0.05);观察组对护理技术的满意度评分是(94.35±2.23)分,对服务态度的满意度评分是(92.58±3.23)分,对沟通交流的满意度评分是(94.44±2.88)分,显著比对照组高,差异有统计学意义(t=15.331、12.123、13.363,P<0.05);观察组患者的并发症发生率是4.76%(2例),对照组患者的并发症发生率是21.43%(9例),两组数据组间差异有统计学意义(χ^(2)=5.126,P<0.05)。结论个体化饮食护理用于肺结核并糖尿病患者可以稳定血糖水平,减少血糖波动,对病情产生积极的影响作用,显著改善患者生活品质,减少并发症,值得临床使用。
Objective To analyze the impact of individualized dietary care on blood glucose levels in patients with tuberculosis and diabetes. Methods The 84 patients with tuberculosis and diabetes who were treated in the hospital from February 2018 to August 2020 were selected for discussion. The patients were divided into two groups in chronological order, and the patients receiving routine intervention belonged to the control group, and the combined individualized diet care patients belonged to the observation group. The blood glucose and its fluctuations and quality of life were compared between the two groups.Results The fasting blood glucose of the observation group(6.87±0.48)mmol/L, the 2 h postprandial blood glucose(8.34±0.37)mmol/L, glycosylated hemoglobin(6.33±0.26)%, lower than the control group, the difference was statistically significant(t=9.149, 16.164, 14.044, P<0.05);the 24 h blood glucose fluctuation value of the observation group after three months of discharge was(2.03±0.24) mmol/L, and the fasting blood glucose variation coefficient(14.27±3.98)% was lower than that of the control group(t=23.331, 9.712, P<0.05);the scores of physical quality, mental health, role and social function of the observation group were(76.74±3.23)points,(72.08±3.27)points,(78.18±3.58)points, and(75.26±3.66)points, respectively, which were higher than those of the control group,the difference was statistically significant(t=8.306, 6.909, 7.641, 7.055, P<0.05);the observation group’s satisfaction score for nursing technology was(94.35±2.23)points, and the satisfaction score for service attitude was(92.58 ±3.23)points, and the communication satisfaction was(94.44 ±2.88)points, which was significantly higher than the control group, the difference was statistically significant(t=15.331, 12.123, 13.363, P<0.05);the complication rate of the observation group was 4.76%(2 cases), and the control group’s complications incidence was 21.43%(9 cases),and the difference between the two data groups was statistically significant(χ^(2)=5.126, P<0.05). Conclusion Individualized diet care for patients with tuberculosis and diabetes can stabilize blood glucose levels, reduce blood glucose fluctuations,have a positive effect on the condition, significantly im prove the quality of life of patients, reduce complica tions, and is worthy of clinical use.
