MIPPO技术在肱骨近端骨折钢板内固定术中的应用效果

Effect of MIPPO Technique in Internal Fixation of Proximal Humeral Fractures with Steel Plate

目的分析肱骨近端骨折患者采取MIPPO技术钢板内固定治疗的临床疗效与应用价值。方法选取2017年7月—2021年7月溧水区中医院收治的66例肱骨近端骨折患者为研究对象,以数表法划分观察组(33例)与对照组(33例)。对照组患者实施传统切开复位内固定术治疗,观察组患者采取MIPPO技术联合钢板内固定手术治疗。记录对比两组患者手术临床指标,观察评估各组患者治疗前后肩关节功能变化,统计各组患者术后并发症总发生率。结果观察组患者手术时长(95.35±11.04)min、患者骨折愈合时间(11.47±1.03)周、住院时间(14.24±1.55)d均明显短于对照组,同时术中出血量(120.25±11.17)mL少于对照组,切口长度(9.10±1.25)cm短于对照组,差异有统计学意义(t=11.026、12.126、15.386、68.711、16.346,P<0.05)。术后观察组患者肩关节上举、外展上举、体侧外旋、体侧内旋功能评分均明显高于对照组,差异有统计学意义(P<0.05)。另外,观察组患者术后并发症总发生率(3.03%)低于对照组,差异有统计学意义(χ^(2)=8.836,P<0.05)。结论肱骨近端骨折患者通过采取MIPPO联合钢板内固定术治疗具有切口小、术后患者恢复快等优势,同时可促进患者肩关节功能的快速康复,安全性高,提倡临床应用推广。

Objective To analyze the clinical efficacy and application value of MIPPO technique steel plate internal fixation in patients with proximal humeral fractures.Methods Selected 66 patients with proximal humeral fractures were admitted to the Lishui District Hospital of Traditional Chinese Medicine from July 2017 to July 2021 as the study subjects.The observation group(33 cases)and the control group(33 cases)were divided by number table.Patients in the control group were treated with traditional open reduction and internal fixation,and patients in the observation group were treated with MIPPO technology combined with steel plate internal fixation.The clinical indicators of the two groups were recorded and compared,and the changes in shoulder joint function before and after treatment in each group were observed and evaluated.The total incidence of postoperative complications in the group were collected.Results The operation time of the observation group was(95.35±11.04)min,the fracture healing time(11.47±1.03)weeks,and the hospitalization time(14.24±1.55)d were significantly shorter than those in the control group.At the same time,the intraoperative blood loss was(120.25±11.17)mL less than the control group,the incision length(9.10±1.25)cm was shorter than the control group,the difference was statistically significant(t=11.026,12.126,15.386,68.711,16.346,P<0.05).The function scores of shoulder uplift,abduction uplift,lateral external rotation,and lateral internal rotation of the observation group were significantly higher than those of the control group,the difference was statistically significant(P<0.05).In addition,the total incidence of postoperative complications in the observation group was 3.03% lower than that in the control group,the difference was statistically significant(χ^(2)=8.836,P<0.05).Conclusion MIPPO combined with plate internal fixation for patients with proximal humeral fractures has the advantages of small incision and quicker recovery after surgery.At the same time,it can promote the rapid recovery of the shoulder joint function of patients with high safety.It is recommended to promote clinical application.
