保乳手术治疗老年早期乳腺癌的疗效及对患者生活质量的影响

Breast-conserving Surgery in the Treatment of Early Breast Cancer in the Elderly and Its Impact on the Quality of Life of Patients

目的探讨保乳手术治疗老年早期乳腺癌的疗效及对患者生活质量的影响。方法选取2019年3月—2020年12月该院收治的86例老年早期乳腺癌患者,按照随机数表法分两组,各43例。研究组行保乳手术治疗,参照组行传统改良根治术治疗,比较两组乳房美观效果、手术相关指标、并发症发生率及治疗前后生活质量变化。结果研究组乳房美观优良率为88.37%,高于参照组的58.14%,差异有统计学意义(χ^(2)=10.030,P<0.05)。研究组手术时间、住院时间均短于参照组,术中出血量少于参照组,差异有统计学意义(P<0.05)。术后,研究组乳腺癌生存质量量表(FACT-B)评分高于参照组,差异有统计学意义(P<0.05)。研究组并发症总发生率为4.65%,低于参照组的20.93%,差异有统计学意义(P<0.05)。结论保乳手术治疗老年早期乳腺癌具有快捷、微创、美观、安全及术后恢复快等优点,对患者生活质量提升有重要意义。

Objective To explore the curative effect of breast-conserving surgery in the treatment of early breast cancer in the elderly and its impact on the quality of life of patients.Methods A total of 86 elderly patients with early breast cancer admitted to the hospital from March 2019 to December 2020 were selected and divided into two groups according to the random number table method,with 43 cases in each group.The study group underwent breast-conserving surgery,and the reference group underwent traditional modified radical mastectomy.The two groups were compared with breast aesthetics,surgery-related indicators,complication rates,and changes in quality of life before and after treatment.Results The excellent and good rate of breast beauty in the study group was 88.37%,which was higher than 58.14%in the reference group,the difference was statistically significant(χ^(2)=10.030,P<0.05).The operation time and hospital stay of the study group were shorter than those of the reference group,and the intraoperative blood loss was less than that of the reference group.The difference was statistically significant(P<0.05).After operation,the breast cancer quality of life scale(FACT-B)score of the study group was higher than that of the reference group,and the difference was statistically significant(P<0.05),the total incidence of complications in the study group was 4.65%,which was lower than 20.93%in the reference group,and the difference was statistically significant(P<0.05).Conclusion Breast-conserving surgery for the treatment of early breast cancer in the elderly has the advantages of rapid,minimally invasive,beautiful,safe and quick recovery after surgery,and is of great significance to the improvement of the quality of life of patients.
