摘要
目的回顾性分析32例口腔颌面颈部重型多间隙感染临床特点,为该病的规范治疗提供依据。方法收集口腔颌面颈部重型多间隙感染病例资料。研究内容包括一般资料、病因学、系统性疾病、病原菌、影像学、治疗方法及并发症。结果32例患者中男性18例(56.2%),女性14例(43.8%);糖尿病患者23例(71.9%)。牙源性感染为最常见感染来源(46.9%)。疼痛、面颈部肿胀、吞咽疼痛、发热、开口受限等是常见的临床症状。28例行病原菌检测患者中,溶血性链球菌9例(32.1%)。颌下间隙是最常见受累间隙。28例行脓肿切开引流,4例行保守治疗。其中2例死亡。结论对于口腔颌面颈部重型多间隙感染,通过多学科的综合治疗可取得良好效果。早期脓肿切开引流是首选的治疗手段。
Objective To retrospectively analyze the clinical characteristics of 32 cases with severe multiple space infections in oral and maxillofacial region,in order to provide basis for the standardized treatment of the disease.Methods Data were collected of patients with severe oral and maxill of acial space infections in the Department of Oral and Maxill of acial Surgery.Their demography,etiology,associated systemic diseases,bacteriology,radiology,treatment methods and complications were reviewed.Results Thirty-two patients with 18 males and 14 females were reviewed,with 23 patients having diabetes mellitus(DM).Odontogenic infection was the most common infection source(46.9%).Pain,swelling,odynophagia,fever and trismus were the most common presenting symptom.Among 28 patients undergoing pathogen detection,9 cases(32.1%)were with hemolytic streptococcus.The submaxillary space was the most commonly involved space.Surgical intervention was conducted on 28 patients,whereas 4 patients were treated with antibiotic therapy alone,and there were 2 patient deaths.Conclusion Patients with severe oral and maxill of acial space infections received fine results through multidisciplinary treatment,and incision and drainage of early abscess was the preferred treatment.
