不同胰岛素方案对Wilste入路手术治疗胸腰椎骨折合并糖尿病患者血糖影响对比

Comparison of the Effects of Different Insulin Regimens on the Wilste Approach Surgical Treatment of Thoracolumbar Fractures with Diabetes Mellitus

目的探讨对于患骨折伴随糖尿病的患者进行不同胰岛素方案,对其手术治疗以及血糖的影响。方法选取该院在2019年7月—2021年7月所接收和治疗的糖尿病合并胸腰椎骨折的患者共60例,按照随机法分为两组,每组30例,分别为治疗组和对照组。两组患者均实施Wilste入路手术治疗。其中对照组主要对患者进行常规胰岛素治疗,治疗组则对患者进行中短效胰岛素联合长效胰岛素控制血糖。对比两组患者的治疗效果、术后恢复情况以及对血糖的影响。结果治疗组的患者在治疗后的血糖指标明显优于对照组,差异有统计学意义(P<0.05)。且治疗组的创面愈合时间、骨折愈合时间明显优于对照组,差异有统计学意义(P<0.05)。治疗组发生并发症的概率明显低于对照组,差异有统计学意义(P<0.05)。结论对于胸腰椎骨折合并糖尿病的患者,对其进行Wilste入路手术中,中短效胰岛素联合长效胰岛素控制血糖对于糖尿病的血糖改善较大,术后血糖水平较为平稳,有利于控制血糖。

Objective To explore the effects of different insulin regimens on patients with fractures and diabetes, their surgical treatment and blood sugar. Methods A total of 60 patients with diabetes and thoracolumbar fractures received and treated by the hospital from July 2019 to July 2021 were selected. According to the random method, they were divided into two groups, 30 cases in each group, respectively, the treatment group and the control group. Both groups were treated with Wilste approach surgery. In the control group, patients were treated with conventional insulin, and the treatment group was treated with short-and medium-acting insulin combined with long-acting insulin to control blood sugar. The treatment effect, postoperative recovery and the effect on blood sugar of the two groups of patients were compared. Results The blood glucose indexes of patients in the treatment group were significantly better than those in the control group after treatment,and the difference was statistically significant(P<0.05). And the wound healing time, fracture healng time of the treatment group were significantly better than those of the control group, and the difference was statistically significant(P<0.05). The probability of complications in the treatment group was significantly lower than that of the control group, and the difference was statistically significant(P<0.05). Conclusion For patients with thoracolumbar fractures and diabetes mellitus, Wilste approach surgery is performed on is light, and the combination of short-and medium-acting insulin combined with longacting insulin to control blood sugar can greatly improve the blood sugar of diabetes. The postoperative blood glucose level is relatively stable, which is conducive to blood glucose control.
