摘要
目的分析对糖尿病合并脓毒症的患者发生器官衰竭及ICU病死率的影响因素。方法选取该院在2020年7月—2021年7月收治的60例糖尿病合并脓毒症患者作为研究对象,测验所有患者的糖化血红蛋白(HbA1c),按照其结果将所有患者分为升高组(HbA1c≥6.5%)和正常组(HbA1c<6.5%),每组30例,观察两组患者的一般基线资料、发生器官功能障碍以及ICU病死率,并分析发生器官功能障碍进展的相关影响因素和ICU死亡的独立影响因素。结果升高组发生器官供能障碍及ICU病死率的概率明显高于正常组,差异有统计学意义(P<0.05)。且经过分析显示,HbA1c≥6.5%、血乳酸均是发生器官功能衰竭的独立影响因素,而其中HbA1c≥6.5%、血乳酸及年龄均是ICU死亡的独立影响因素。结论对于糖尿病合并脓毒症的患者,其HbA1c的检验对评估器官功能障碍及ICU病死率均有一定的预测价值,且其中HbA1c≥6.5%、血乳酸及年龄是影响两者的主要因素。
Objective To investigate and analyze the influencing factors of organ failure and ICU mortality in patients with diabetes mellitus complicated with sepsis. Methods A total of 60 patients with diabetes and sepsis received and treated by the hospital from July 2020 to July 2021 were selected as the research objects, and the glycosylated hemoglobin(HbA1c) of all patients was tested. According to the results, all patients were divided into the elevated group(HbA1c≥6.5%) and the normal group(HbA1c<6.5%), with 30 cases in each group. General baseline data, developing organ dysfunction, and ICU case fatality rates were observed in both groups, and the relevant influencing factors of developing organ dysfunction progression and the independent influencing factors of ICU death were analyzed. Results The probability of organ energy failure and ICU mortality in the elevated group was significantly higher than that of the normal group, and the difference was statistically significant(P <0.05). And the analysis showed that HbA1c ≥6.5% and blood lactate were independent influencing factors of organ failure, and HbA1c ≥6.5%, blood lactate and age were all independent influencing factors of ICU death. Conclusion For patients with diabetes complicated with sepsis, the HbA1c test has a certain predictive value in assessing organ dysfunction and ICU mortality. Among them, HbA1c ≥6.5%, blood lactate and age are the main factors affecting both.
