糖尿病肾病早期生化检验的诊断效果及血清hs-CRP指标水平影响分析 被引量:12

摘要 目的分析糖尿病肾病早期生化检验的诊断效果及对血清超敏C反应蛋白(hs-CRP)指标水平影响。方法抽取2020年3月—2021年3月50例糖尿病肾病患者作为研究组,同期选择50名健康体检者作为对比组,两组均进行早期生化检验,分析检验结果。结果研究组hs-CRP、血清胱抑素C(CysC)、糖化血红蛋白(HbA1c)、血肌酐(Cr)血液生化检验指标水平以及尿糖(GLU)、尿微量白蛋白(mAlb)尿液生化检验指标水平与对照组比较,差异有统计学意义(P<0.05);血液生化检验联合尿液生化检验准确率98.00%、灵敏度89.88%以及特异度82.45%等均较单一检验(血液生化检验:82.00%、73.12%、70.26%;尿液生化检验:80.00%、74.25%、71.37%)更高,差异有统计学意义(P<0.05)。结论在糖尿病肾病患者临床诊断中,进行早期生化检验,通过分析hs-CRP、CysC、Hb A1c、Cr、GLU,m Alb等相关指标,与正常范围进行比较,结合临床症状、表现等,能够对糖尿病肾病进行准确诊断,但单一进行血液生化检验或者尿液生化检验,准确率、灵敏度以及特异度等不是非常理想。因此,在糖尿病肾病患者临床诊断中需采取血液生化检验联合尿液生化检验方式,以期提高整体诊断效能。 Objective To analyze the diagnostic effect of early biochemical tests for diabetic nephropathy and its influence on serum hs-CRP index levels. Methods 50 cases of diabetic nephropathy from March 2020 to March 2021 were selected as the research group, and 50 healthy subjects were selected as the comparison group during the same period. Both groups were subjected to early biochemical tests to analyze the test results. Results There were statistically significant differences in hs-CRP, serum cystatin C(CysC), glycosylated hemoglobin(HbA1c), serum creatinine(Cr), urine glucose(GLU) and urine microalbumin(mAlb) between the study group and the control group(P<0.05). The accuracy, sensitivity and specificity of blood biochemical test combined with urine biochemical test were 98.00%, 89.88% and 82.45% etc., which were better than single test(blood biochemical test:82.00%, 73.12%, 70.26%;urine biochemical test: 80.00%, 74.25%, 71.37%)higher,the difference was statistically significant(P <0.05). Conclusion In the clinical diagnosis of diabetic nephropathy, early biochemical tests are performed, by analyzing related indicators such as hs-CRP, Cys C, HbA1c, Cr, GLU, mAlb, and comparing with the normal range, combined with clinical symptoms and manifestations, it can accurately diagnose diabetic nephropathy. However, the accuracy, sensitivity, and specificity of a single blood biochemical test or urine biochemical test are not very ideal. Therefore, in the clinical diagnosis of diabetic nephropathy patients, it is necessary to adopt blood biochemical test combined with urine biochemical test to improve the overall diagnostic efficiency.
