双极人工股骨头置换术治疗老年股骨颈骨折的临床效果分析

Analysis of the Clinical Effect of Bipolar Artificial Femoral Head Replacement in the Treatment of Femoral Neck Fractures in the Elderly

目的分析研究双极人工股骨头置换术治疗老年股骨颈骨折的临床效果。方法选取该院2017年1月—2021年1月收治的老年股骨颈骨折患者100例,遵循随机原则进行分组,对照组50例,观察组50例。对照组给予常规治疗,观察组在对照组基础上加用双极人工股骨头置换术治疗,以两组患者的治疗效果、并发症发生率、各项手术指标、生活质量、髋关节功能为观察指标,然后统计比较观察结果。结果分析治疗效果,和对照组患者的治疗有效率(82.00%)相比,观察组患者的治疗有效率(98.00%)更高,差异有统计学意义(χ^(2)=7.111,P<0.05)。在并发症方面,和对照组患者的并发症发生率(18.00%)相比,观察组患者的并发症发生率(2.00%)更低,差异有统计学意义(P<0.05)。观察组下床活动时间优于对照组,差异有统计学意义(P<0.05)。观察组生活质量评分较对照组更高,差异有统计学意义(P<0.05)。观察组髋关节功能评分较对照组更高,差异有统计学意义(P<0.05)。结论实施双极人工股骨头置换术治疗方法,对老年股骨颈骨折患者具有降低并发症发生率、提高髋关节功能的效果,且缩短术后下床活动时间,髋关节功能随之提升,很大程度上提高了治疗水平,在临床中值得应用和推广。

Objective To analyze and study the clinical effect of bipolar artificial femoral head replacement in the treatment of elderly femoral neck fractures.Methods The research subjects selected 100 elderly patients with femoral neck fractures admitted to the hospital.The research period was selected from January 2017 to January 2021.Followed the random principle to group,50 cases in the control group and 50 cases in the observation group.The control group was given conventional treatment,and the observation group was treated with bipolar artificial femoral head replacement on the basis of the control group.The treatment effect,complication rate,various surgical indicators,quality of life,and hip joint function of the two groups of patients were used as observation indicators.Then statistically compared the observation results.Results Analysis of the treatment effect,compared with the treatment effective rate of the control group(82.00%),the observation group had a higher treatment effective rate(98.00%),the difference was statistically significant(χ^(2)=7.111,P<0.05).In terms of complications,compared with the complication rate of the control group(18.00%),the observation group had a lower complication rate(2.00%),the difference was statistically significant(P<0.05).The out of bed activity time of the observation group were better than the control group,the difference was statistically significant(P<0.05).The quality of life score of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).The hip joint function score of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion The implementation of bipolar artificial femoral head replacement has the effect of reducing the incidence of complications and improving hip joint function in elderly patients with femoral neck fractures.It also shortened the time to get out of bed after the operation,and the hip joint function was improved.To a large extent,the level of treatment has been improved.It is worthy of application and promotion in clinical practice.
