DIP视角下医院感染管理质量的经济效益分析

Economic Benefit Analysis of Hospital Infection Management Quality from DIP Perspective

目的在按病种分值付费(DIP)模式下,分析医院感染管理质量的经济效益,为各医疗机构重视推进和提升医院感染管理水平提供依据。方法对10653名出院患者按照是否发生医院感染分组并进行回顾性分析。结果发生医院感染的出院患者平均住院费用(36713.30元)和平均住院天数(22.63 d)显著高于未发生医院感染的出院患者组;每例医院感染将导致直接损失29674.57元,每投入1元感控费用,将减少82元的直接损失;医院感染发生率与住院收入利润率(或降低成本率)存在直接关系;医院感染例数与住院患者数量正相关。结论医院感染管理质量影响着DIP模式下医保资金的最终核算结果,提升医院感染管理质量,可以有效降低额外损失,提高医院经济效益,推动医院高质量发展。

Objective:The economic benefits of hospital infection management quality were analyzed under the pay-per-disease(DIP)mode,which provided a basis for medical institutions to attach importance to promoting and improving hospital infection management.Method:10,653 discharged patients were grouped according to hospital infection and analyzed retrospectively.Result:The average hospitalization cost(36,713.30 yuan)and average length of stay(22.63 days)of discharged patients with hospital infection were significantly higher than those without hospital infection.Each hospital infection will lead to a direct loss of 29,674.57 yuan,and every 1 yuan of infection control cost will reduce the direct loss of 82 yuan.The incidence of hospital infection is directly related to the profit rate of hospitalization revenue(or cost reduction rate).The number of hospital infection cases was positively correlated with the number of inpatients.Conclusion:The quality of hospital infection management affects the final accounting results of medical insurance funds under DIP mode.Improving the quality of hospital infection management can effectively reduce additional losses,improve the economic benefits of the hospital and promote the high-quality development of the hospital.
