福建省2009—2019年输入性卵形疟及亚型的回顾性分析

Retrospective analysis on imported plasmodium ovale and its subspecies in Fujian,2009-2019

摘要 目的分析福建省2009—2019年输入性卵形疟及亚型curtisi和wallikeri的流行分布,加强防治措施降低危害。方法收集聚合酶链式反应(PCR)确诊的80例输入性卵形疟及个案信息资料,回顾性分析输入性卵形疟及亚型的流行分布。结果福建省2009—2019年卵形疟检出率呈增高趋势(趋势χ^(2)=14.07,P<0.01),年均检出率8.2%(80/978),2015年检出率最高,达18.6%;80例卵形疟中,其中curtisi亚型28例(35.0%)、wallikeri亚型37例(46.2%)、二者混合感染15例(18.8%);感染来源地区分布:来自西非者32例(40.0%),其中curtisi16例、wallikeri 11例、混合感染5例;来自中非者33例(41.3%),其中curtisi 9例、wallikeri 18例、混合感染6例;来自东非者15例(18.7%),其中curtisi 3例、wallikeri8例、混合感染4例;复发率为5.0%(4/80),从国内首次发作到复发,4个病例分别间隔22 d、35 d、180 d和28 d。结论福建省2009—2019年输入性卵形疟呈增高趋势,两个亚型的地理分布散在、广泛,无明显区别。 Objective To analyze the epidemiological distribution of imported plasmodium ovale and its subspecies curtisi and wallikeri in Fujian from 2009 to 2019 for the disease control and prevention.Methods Totally 80 blood samples of imported plasmodium ovale and its subspecies cases confirmed by PCR method and the individual information were collected and the epidemiological characteristics were analyzed retrospectively.Results The positive rate of plasmodium ovale from 2009 to 2019 showed increasing tendency(trendχ^(2)=14.07,P<0.01)and mean annual positive rate was 8.2%(80/978)and the peak rate was in 2015 with 18.6%.In 80 plasmodium ovale cases,there were 28 curtisi cases(35.0%),37 wallikeri cases(46.2%)and 15 mix infection cases(18.8%).The areas of infection source:32 plasmodium ovale cases(40.0%)from western Africa,with 16 curtisi cases,11 wallikeri cases and 5 mix infection cases;33 plasmodium ovale cases(41.3%),from middle Africa,with 9 curtisi cases,18 wallikeri cases and 6 mix infection cases;15 plasmodium ovale cases(18.7%)from eastern Africa,with 3 curtisi cases,8 wallikeri cases and 4 mix infection cases.The recurrence rate of the cases was 5.0%(4/80).The interval days for the four recurrence cases from first episode to first recurrence in Fujian were 22 days,35 days,180 days and 28 days.Conclusion The tendency of imported plasmodium ovale cases is increasing in Fujian from 2009 to 2019.There is no obvious difference of geographical distribution with scatteredly and extensively for their two subtypes.
