集束化护理预防重度颅脑损伤气管插管患者VAP发生的作用

Effect of Cluster Nursing in Prevention of Occurrence of VAP in Severe Craniocerebral Injury Patients with Tracheal Intubation

目的探讨集束化护理预防重度颅脑损伤气管插管患者呼吸机相关性肺炎(VAP)的作用。方法437例行气管插管的重度颅脑损伤患者分为观察组(集束化护理)与对照组(常规护理),比较两组的护理效果。结果观察组的VAP发生率低于对照组,机械通气时间、住ICU时间均短于对照组(P<0.05)。干预后,观察组的PaO_(2)、SaO_(2)水平均高于对照组,PaCO_(2)水平低于对照组(P<0.05)。观察组的社会生活、躯体活动、睡眠情况、精力评分均高于对照组(P<0.05)。结论集束化护理可有效预防重度颅脑损伤气管插管患者VAP的发生,促进患者恢复,改善患者生活质量。

Objective To explore the effect of cluster nursing in prevention of ventilator-associated pneumonia(VAP)in severe craniocerebral injury patients with tracheal intubation.Methods 437 cases of severe craniocerebral injury patients with tracheal intubation were divided into the observation group(cluster nursing)and the control group(regular nursing).The nursing effects of the two groups were compared.Results The incidence of VAP in the observation group was lower than that in the control group,and the mechanical ventilation time and ICU staying time were shorter than those in the control group(P<0.05).After intervention,the levels of PaO_(2) and SaO_(2) in the observation group were higher than those in the control group,and the level of PaCO_(2) was lower than that in the control group(P<0.05).The scores of social life,physical activity,sleeping status and energy in the observation group were higher than those in the control group(P<0.05).Conclusions Cluster nursing can effectively prevent the occurrence of VAP in severe craniocerebral injury patients with tracheal intubation,promote the recovery of patients,and improve the quality of life of patients.
