激光改良传统术式对单纯下肢静脉曲张的疗效

Effect of laser-modified traditional surgery on simple lower extremity varicose veins

目的探讨激光改良传统术式——高位结扎剥脱术联合激光腔内闭合术(EVLT)对单纯下肢静脉曲张的疗效。方法收集2018年1月至2019年1月昆山市第一人民医院收治的单纯下肢静脉曲张患者临床资料,根据治疗方法不同将患者分为激光改良组(采用高位结扎剥脱术联合EVLT治疗)和传统组(采用高位结扎剥脱术),每组40例。比较两组患者的手术时间,住院时间,术中出血量,术后疼痛程度,术后患肢皮肤麻木、皮肤瘀斑、切口感染发生率,术后1年患肢静脉曲张复发率。结果激光改良组患者的手术时间、住院时间均短于传统组患者,术中出血量和视觉模拟评分法(VAS)评分均少于传统组患者,差异均有统计学意义(P<0.01)。激光改良组患者术后患肢皮肤麻木、皮肤瘀斑、切口感染的发生率及术后1年下肢静脉曲张的复发率均低于传统组患者,差异均有统计学意义(P<0.01)。结论激光改良传统术式治疗下肢静脉曲张的临床疗效显著,弥补了传统术式和EVLT的不足。

Objective To explore the curative effect of laser-modified traditional surgery--high ligation and stripping combined with endovenous laser treatment(EVLT)on simple lower extremity varicose veins.Method The clinical data of patients with simple lower extremity varicose veins admitted to First People’s Hospital of Kunshan from January 2018 to January 2019 were collected.The patients were divided into the laser-modified group(using high ligation and stripping combined with EVLT)and the traditional group according to varying treatment methods(using high ligation and stripping)with 40 cases in each group.The surgery time,hospitalization time,intraoperative blood loss volume,postoperative pain,the incidence rate of postoperative skin numbness,the incidence rate of skin ecchymosis,the incidence rate of incision infection,and recurrence rate of the lower extremity varicose veins were compared between the two groups of patients.Result The operative time and hospital stay of the laser-modified group were shorter than those of the traditional group,intraoperative blood loss volume and visual analogue scale(VAS)score of the laser-modified group were lower than those of the traditional group,the differences were statistically significant(P<0.01).The incidence rate of postoperative skin numbness,skin ecchymosis,and incision infection of the patients in the laser-modified group and the recurrence rate of the lower extremity varicose veins 1 year after surgery were lower than those in the traditional group,and the differences were statistically significant(P<0.05).Conclusion Laser-modified traditional surgery has significant clinical effects in the treatment of lower extremity varicose veins,which fills in the shortcomings of traditional surgery and EVLT.
