可回收式下腔静脉滤器的回收成功情况分析

Analysis of successful retrieval of the retrievable inferior vena cava filter

目的探讨可回收式下腔静脉滤器(IVCF)的回收成功率及影响滤器回收成功率的相关因素。方法收集2016年5月至2019年12月在喀什地区第一人民医院住院治疗并行下腔静脉滤器置入术的114例急性下肢深静脉血栓形成(DVT)患者的临床资料。观察所有急性下肢DVT患者的滤器置入与回收情况。分析IVCF回收成功情况与急性下肢DVT患者临床特征的关系。结果共114例急性下肢DVT患者接受尝试IVCF回收,滤器回收成功率为80.7%(92/114)。中央型急性下肢DVT患者的滤器回收成功率低于混合型及外周型下肢DVT,D-二聚体水平高的急性下肢DVT患者的滤器回收成功率低于D-二聚体水平低的患者,差异均有统计学意义(P<0.05)。不同年龄、性别、DVT位置、DVT类型、IVCF类型、合并肺动脉栓塞(PE)情况、行导管接触性溶栓治疗情况、适应证、滤器取出年限、口服抗凝药物情况、DVT时间、滤器滞留时间、国际标准化比值、血小板计数的急性下肢DVT患者的IVCF回收成功率比较,差异均无统计学意义(P>0.05)。结论中央型下肢DVT患者的滤器回收成功率低于外周型和混合型下肢DVT患者。滤器回收前的D-二聚体水平越高,滤器的回收成功率越低。

Objective To investigate the retrieval successful rate of retrievable inferior vena cava filter(IVCF)and the related factors affecting the recovery of the filter.Method The clinical data of 114 patients with acute lower extremity deep venous thrombosis(DVT)hospitalized in the First People's Hospital of Kashi District from May 2016 to December 2019 were collected.The filter placement and recovery of all patients with acute lower extremity DVT were observed.To analyze the relationship between the success of IVCF recovery and the clinical characteristics of patients with acute lower extremity DVT.Result A total of 114 patients with acute lower extremity DVT underwent IVCF recovery.The success rate of filter recovery was 80.7%(92/114).The success rate of filter recovery in patients with central acute lower extremity DVT was lower than that in patients with mixed and peripheral lower extremity DVT.The success rate of filter recovery in patients with high D-dimer level was lower than that in patients with low D-dimer level(P<0.05).There was no significant difference in IVCF recovery rate among patients with acute lower extremity DVT of different age,gender,DVT location,DVT type,IVCF type,pulmonary embolism(PE),CDT treatment,indications,filter removal years,oral anticoagulants,DVT time,filter retention time,international standardized ratio and platelet count(P>0.05).Conclusion The success rate of filter recovery in patients with central acute lower extremity DVT is lower than that in patients with mixed and peripheral lower extremity DVT.The higher the D-Dimer level is before filter recovery,the lower is the recovery success rate of filter.
