摘要
目的:探究甲状腺乳头状癌(PTC)组织中SET和MYND结构域蛋白2(SMYD2)、脂肪酸2-羟基化酶(FA2H)的表达及其与临床病理特征、预后的关系。方法:收集2013年6月至2015年2月我院手术治疗的PTC患者的组织样本(癌组织和癌旁正常组织),实时荧光定量PCR(RT-PCR)检测组织样本中SMYD2、FA2H mRNA表达情况;以SMYD2、FA2H mRNA表达水平的中位数为界,分为高表达组和低表达组,分析SMYD2、FA2H表达与PTC临床病理特征及预后的关系;多因素Cox回归模型分析PTC患者预后影响因素。结果:与癌旁正常组织相比,PTC癌组织中SMYD2、FA2H mRNA相对表达量增加(均P<0.05)。PTC癌组织中,SMYD2表达与肿瘤侵犯包膜、浸润深度及临床分期相关(均P<0.05);FA2H表达与肿瘤直径、淋巴结转移及临床分期相关(均P<0.05)。SMYD2高表达组与低表达组患者的5年总生存率(OS)、5年无病生存率(RFS)比较,差异有统计学意义(均P<0.05);FA2H高表达组与低表达组患者的5年OS、5年RFS比较,差异有统计学意义(均P<0.05)。多因素Cox回归模型分析结果表明,SMYD2表达水平、FA2H表达水平、淋巴结转移及临床分期是影响PTC患者预后的危险因素。结论:PTC癌组织中SMYD2、FA2H均呈高表达状态,且均与较差临床病理特征、不良预后相关;SMYD2、FA2H可能是预测PTC患者预后的潜在指标。
Objective:To explore the relationships between SET and MYND domain containing protein 2(SMYD2),fatty acid 2-hydroxylase(FA2H)expressions with the clinicopathological parameters and prognosis in papillary thyroid carcinoma(PTC)cancer tissues.Methods:The tissue samples of PTC patients who came to our hospital for surgical resection from June 2013 to February 2015 were collected(cancer tissues and adjacent normal tissues),the real-time fluorescence quantitative PCR(RT-PCR)was used to detect the mRNA expressions of SMYD2 and FA2H in tissue samples.The PTC patients were divided into the high expression group and low expression group by the medium SMYD2 and FA2H mRNA expression level.The correlation between the SMYD2 and FA2H expressions in PTC cancer tissues and the clinicopathological characteristics and prognosis were analyzd.Multivariate Cox regression model was used to analyze the prognostic factors of PTC patients.Results:Compared to the adjacent normal tissues,the relative mRNA expression levels of SMYD2 and FA2H were both increased in PTC cancer tissues(both P<0.05).In the PTC cancer tissues,the expression of SMYD2 was related with tumor capsular invasion,invasion depth and the clinical stage(all P<0.05),the expression of FA2H was related with tumor diameter,lymph node metastasis and the clinical stage(all P<0.05).There was a significant difference in 5-year over-all survival rate(OS)and 5-year relapse-free survival rate(RFS)between the high-SMYD2 group and low-SMYD2 group(both P<0.05);there was a significant difference in 5-year OS and 5-year RFS between the FA2H high group and FA2H low group(both P<0.05).Cox's proportional hazards regression model results suggested that SMYD2 expression,FA2H expression,lymph node metastasis and the clinical stage are the risk factors of PTC prognosis(all P<0.05).Conclusion:In PTC cancer tissues the SMYD2 and FA2H expression are up-regulated,which is related to worse clinicopathological parameters and poorer prognosis.The SMYD2 and FA2H may be potential prognostic indicators for PTC patients.
