鼻咽癌治疗实践的矛盾省思及启示

Paradox Reflection and Enlightenment in the Treatment of Nasopharyngeal Carcinoma

鼻咽癌是我国南方地区最常见的头颈部恶性肿瘤之一,放疗、化疗相结合的综合治疗是其主要治疗手段。虽然鼻咽癌预后较好,但治疗全程中诸如治不治、如何治,如何平衡药的“效”与“毒”,如何平衡病与并发症等矛盾难题的客观存在始终影响、掣肘着临床实践。运用矛盾分析法厘清鼻咽癌治疗实践的矛盾,同时运用具体问题具体分析原理把握患者、病程、治疗的特殊性精准施治,是化解鼻咽癌治疗实践矛盾的关键,对于其他医疗实践活动亦具有重要的借鉴意义。

Nasopharyngeal carcinoma is one of the commonest head and neck malignancies in southern China.Radiotherapy combined with chemotherapy is the primary treatment method for nasopharyngeal carcinoma.Although the prognosis of nasopharyngeal carcinoma is good,paradox always affects and hinders clinical practice,such as whether to treat,how to treat,how to balance the"effect"and"toxicity"of drugs,and how to balance disease and complications,etc.In this study,contradiction analysis method was used to clarify these paradoxes.Case-by-case analysis approach was used for precise treatment in the course of disease due to the particularity,which is the key to resolve the paradox in the treatment practice of nasopharyngeal carcinoma.It is also of significance for other medical practice as a reference.
