彩色多普勒超声对乳腺癌的诊断价值分析

Diagnostic value of color Doppler ultrasound in breast cancer

目的分析彩色多普勒超声对乳腺癌的诊断价值。方法60例乳腺癌患者,均实施彩色多普勒超声诊断,以病理检查为金标准,观察彩色多普勒超声诊断对乳腺癌不同病理类型的检出率,分析彩色多普勒超声诊断乳腺癌患者的特征。结果病理检查显示:浸润性导管癌17例,髓样癌7例,浸润性小叶癌9例,早期导管内癌8例,黏液癌10例,炎性乳癌6例,腺癌3例。彩色多普勒超声对浸润性导管癌的诊断符合率为94.12%,髓样癌的诊断符合率为100.00%,浸润性小叶癌的诊断符合率为88.89%,早期导管内癌的诊断符合率为87.50%,黏液癌的诊断符合率为100.00%,炎性乳癌的诊断符合率为100.00%,腺癌的诊断符合率为100.00%,总诊断符合率为95.00%。60例乳腺癌患者中,形态不规则、呈现锯齿样患者51例,占比85.00%;边界模糊不清、呈毛刺样患者24例,占比40.00%;后方回声较弱且不均匀患者50例,占比83.33%;病灶在外上象限患者47例,占比78.33%;病灶在外下象限患者10例,占比16.67%;病灶在内上象限患者2例,占比3.33%;病灶在内下象限患者1例,占比1.67%;纵横比(L/T)>1患者27例,占比45.00%。结论彩色多普勒超声在乳腺癌的临床诊断中能够详细观察患者的病灶位置,清晰观察患者病灶形态及回声等,能有效提高诊断符合率,以便患者得到及时治疗,值得临床推广和应用。

Objective To analyze the diagnostic value of color Doppler ultrasound in breast cancer.Methods 60 patients with breast cancer were diagnosed with color Doppler ultrasound.The pathological examination was considered as the gold standard.The detection rate of color Doppler ultrasound in different pathological types of breast cancer was collected.The characteristics of patients with breast cancer diagnosed by color Doppler ultrasound were analyzed.Results Pathological examination showed 17 cases of invasive ductal carcinoma,7 cases of medullary carcinoma,9 cases of invasive lobular carcinoma,8 cases of early intraductal carcinoma,10 cases of mucinous carcinoma,6 cases of inflammatory breast cancer,and 3 cases of adenocarcinoma.The coincidence rate of color Doppler ultrasound in the diagnosis of invasive ductal carcinoma,medullary carcinoma,invasive lobular carcinoma,early intraductal carcinoma,mucinous carcinoma,inflammatory breast cancer,and adenocarcinoma were 94.12%,100.00%,88.89%,87.50%,100.00%,100.00%and 100.00%,and the overall diagnostic coincidence rate was 95.00%.Among the 60 patients with breast cancer,51 cases showed irregular shape with saw toothed,accounting for 85%;24 cases showed blurred boundary with burr-like,accounting for 40.00%;50 cases had weak and uneven posterior echo,accounting for 83.33%;47 cases had lesions in the outer upper quadrant,accounting for 78.33%;10 cases had lesions in the outer lower quadrant,accounting for 16.67%;2 cases had lesions in the inner upper quadrant,accounting for 3.33%;1 case had lesions in the inner lower quadrant,accounting for 1.67%;and 27 cases had a aspect ratio(L/T)>1,accounting for 45.00%.Conclusion In the clinical diagnosis of breast cancer with color Doppler ultrasound,it offers the possibility to observe the location of the patient’s lesion in detail,to clearly observe the shape,echo,etc.of the patient’s lesion.It can effectively improve the diagnostic coincidence rate so that the patient can be treated in time.Thus,it is worthy of promotion in clinical application.
