新诊断2型糖尿病患者采用短期胰岛素泵强化治疗的效果

Effect of short-term intensive insulin pump therapy in patients with newly diagnosed type 2 diabetes mellitus

目的研究新诊断2型糖尿病患者采用短期胰岛素泵强化治疗的临床效果。方法94例新诊断2型糖尿病患者,随机分为参照组和研究组,每组47例。参照组采用胰岛素多次皮下注射治疗,研究组采用短期胰岛素泵强化治疗。比较两组患者治疗前后的血糖[空腹血糖(FBG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)]、胰岛β细胞功能指标[空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、胰岛素分泌指数(HOMA-β)]。结果治疗后,研究组患者FBG(5.25±1.11)mmol/L、2 h PG(7.51±1.12)mmol/L、HbA1c(5.11±1.03)%均低于参照组的(6.55±1.09)mmol/L、(9.33±1.55)mmol/L、(7.05±1.13)%,差异均具有统计学意义(P<0.05)。治疗后,研究组患者的FINS(12.15±3.14)mIU/L、HOMA-IR(1.31±0.15)、HOMA-β(82.87±5.54)%均优于参照组的(15.17±3.11)mIU/L、(1.64±0.28)、(93.64±5.82)%,差异均具有统计学意义(P<0.05)。结论新诊断2型糖尿病患者采用短期胰岛素泵强化治疗效果显著,可有效降低患者的血糖指标,改善其胰岛β细胞功能,具有临床应用价值。

Objective To study the clinical effect of short-term intensive insulin pump therapy in patients with newly diagnosed type 2 diabetes mellitus.Methods 94 patients with newly diagnosed type 2 diabetes mellitus were randomly divided into control group and study group,with 47 cases in each group.Patients in the control group received multiple subcutaneous injections of insulin,while patients in the study group were treated with short-term intensive insulin pump therapy.The blood glucose indexes[fasting blood glucose(FBG),2 h postprandial blood(2 h PG),glycosylated hemoglobin(HbA1c)]and isletβ-cell function indexes[fasting insulin(fins),homeostasis model assessment-insulin resistance(HOMA-IR),homeostasis model assessment-β(HOMA-β)]before and after treatment were compared between the two groups.Results After the treatment,the study group had FBG of(5.25±1.11)mmol/L,2 h PG of(7.51±1.12)mmol/L and HbA1c of(5.11±1.03)%,which were lower than(6.55±1.09)mmol/L,(9.33±1.55)mmol/L and(7.05±1.13)%of the control group,respectively,and the differences were statistically significant(P<0.05).After the treatment,the study group had FINS of(12.15±3.14)mIU/L,HOMA-IR of(1.31±0.15),HOMA-βof(82.87±5.54)%,which were better than(15.17±3.11)mIU/L,(1.64±0.28),(93.64±5.82)%of the control group,and the differences were statistically significant(P<0.05).Conclusion The short-term insulin pump intensive therapy has significant therapeutic effect in patients with newly diagnosed type 2 diabetes mellitus,with effectively reduced blood glucose index and improved isletβ-cell function.Thus,it is worthy of clinical application.
