中西医结合治疗老年骨质疏松性桡骨远端骨折(肾阳虚型)的临床疗效

Clinical effect of integrated traditional Chinese and western medicine in the treatment of senile osteoporotic dis⁃tal radius fracture(kidney yang deficiency type)

目的探讨中西医结合在老年骨质疏松性桡骨远端骨折(肾阳虚型)治疗中的应用价值。方法选择老年骨质疏松性桡骨远端骨折(肾阳虚型)患者80例,按照随机数字表法分为两组,对照组40例患者采用西医常规治疗,观察组40例采用中西医结合治疗,治疗3个月后比较疗效。结果治疗后,观察组患者的关节功能总优良率95.00%高于对照组80.00%,差异有统计学意义(χ^(2)=6.72,P<0.05)。治疗后,观察组患者的血清骨钙素(OCN)、护骨素(OPG)、β-内啡肽(β-EP)、骨密度高于对照组,白介素-8(IL-8)水平低于对照组,差异均有统计学意义(t分别=9.18、7.16、11.06、9.06、-9.26,P均<0.05);观察组患者的Gart-Land-Werley评分系统、中医症状积分低于对照组,差异均有统计学意义(t分别=-7.06、-10.08,P均<0.05);观察组患者的骨折愈合时间、临床症状缓解时间、功能恢复时间均低于对照组,差异均有统计学意义(t分别=14.06、9.18、9.82,P均<0.05)。结论对于老年骨质疏松性桡骨远端骨折(肾阳虚型)患者,采用中西医结合治疗有助于减轻炎症反应,调节骨代谢,促进骨质愈合,改善腕关节功能,提高临床疗效。

Objective To explore the value of integrated traditional Chinese and western medicine in the treatment of senile osteoporotic distal radius fracture(kidney yang deficiency type).Methods Eighty elderly patients with osteoporotic distal radius fracture (kidney yang deficiency type) were randomly divided into two groups,40 patients in the control group were treated with conventional western medicine and 40 patients in the observation group were treated with integrated traditional Chinese and western medicine.The curative effects were compared after 3 months of treatment. Results After treatment,the total excellent and good rate of joint function in the observation group was 95.00%,which was higher than 80.00% in the control group(χ^(2)=6.72,P<0.05).After treatment,serum osteocalcin(OCN),osteoprotegerin(OPG),β- endorphins(β-EP)and bone mineral density of the observation group were higher than those of the control group,and the level of interleukin-8(IL-8)was lower than that of the control group(t=9.18,7.16,11.06,9.06,-9.26,P<0.05). The Gart Land Werley score system and TCM symptom scores of the observation group were lower than those of the con-trol group,and the differences were statistically significant(t=-7.06,-10.08,P<0.05).The fracture healing time,clinical symptom relief time and functional recovery time of the observation group were lower than those of the control group(t= 14.06,9.18,9.82,P<0.05). Conclusion For senile osteoporotic distal radius fracture(kidney yang deficiency type), the treatment of integrated traditional Chinese and western medicine is helpful to reduce inflammatory reaction,regulate bone metabolism,promote bone healing,improve wrist function and clinical curative effect.
