上颌窦内外侧宽度对不植骨上颌窦内提升的成骨影响

Influence of lateral-medial sinus width on the outcomes of sinus elevation without graft via transcrestal approach

目的:评估上颌窦内外侧宽度对不植骨的上颌窦内提升成骨效果的影响。方法:回顾性地分析48例不植骨的骨凿法上颌窦内提升术并同期植入种植体的患者,植入种植体52枚,在术前、术后即刻、术后6个月三个时间点拍摄的锥束状CT进行评估及数据分析,分析新骨生成与上颌窦内外侧径的关系。结果:患者的平均上颌窦宽度为(16.07±4.17)mm,平均剩余骨高度为(6.42±1.17)mm,术中平均提升高度为(4.34±0.93)mm,术后6个月平均新骨生成量为(1.06±0.57)mm,新骨生成与上颌窦宽度呈负相关(P<0.01)。结论:在上颌窦内提升不植骨的术式中,上颌窦内外侧径会影响最终窦内成骨的高度,较小的上颌窦宽度有利新骨的生成。

Objective:This study was aimed to evaluate the influence of lateral-medial sinus width on the outcomes of sinus elevation without graft via transcrestal approach.Methods:All 48 patients with 52 implants inserted in 52 lifted sinuses who received non-grafting osteotome sinus elevation with implant placement simultaneously in our department were recruited for this retrospective study according to inclusion and exclusion criteria.Data were measured from patients'cone beam CT which were taken before,right after and 6 months after the surgery.Then relation between the bone formation height(BFH)residual bone height(RBH),elevation height(EH),height ratio(HR,HR=BFH/EH)and sinus width(SW)were analyzed.Results:The average SW and RBH was(16.07±4.17)mm,(6.42±1.17)mm,respectively.The average EH was(4.34±0.93)mm.Intrasinus BFH was(1.06±0.57)mm at 6 months after surgery.Multivariate analysis showed a negative correlation between SW and BFH,HR(P<0.01).Conclusions:SW will affect bone formation height in sinus elevation without graft via transcrestal approach.Insertion site with narrow sinus width tend to have better intrasinus bone formation.
