摘要
目的探讨冠心病患者Precise-DAPT评分与冠状动脉狭窄程度的相关性。方法选取接受冠状动脉介入治疗的539例冠心病患者作为研究对象,根据术前年龄、肌酐清除率、白细胞计数、血红蛋白水平和既往自发出血史计算Precise-DAPT评分,并依据术中造影结果计算Gensini评分及SYNTAX评分,分析Precise-DAPT评分与Gensini评分、SYNTAX评分的相关性。结果二元Logistic回归分析显示,年龄(OR=1.189,95%CI为1.131~1.249,P<0.001)、心率(OR=1.030,95%CI为1.006~1.055,P=0.015)、SYNTAX评分(OR=1.153,95%CI为1.089~1.220,P<0.001)均为Precise-DAPT评分的独立影响因素;Pearson相关性分析显示,Precise-DAPT评分与Gensini评分(r=0.363,P<0.001)、SYNTAX评分(r=0.501,P<0.001)均呈正相关。结论Precise-DAPT评分与冠状动脉狭窄程度呈正相关,出血风险较高的冠心病患者冠状动脉狭窄程度更加严重,在双联抗血小板治疗持续时间方面矛盾更突出,建议个性化治疗并密切随访。
Objective To explore correlation between Precise-DAPT score and coronary artery stenosis. Methods A total of 539 patients with coronary artery disease who underwent coronary artery intervention were selected as study subjects. Precise-DAPT score was calculated based on preoperative age, creatinine clearance rate, leukocyte count, hemoglobin levels, and history of previous spontaneous bleeding. The Gensini score and the SYNTAX score were calculated based on the angiography, and correlations between Precise-DAPT score and Gensini score, the SYNTAX score were analyzed. Results Binary Logistic regression analysis showed that age(OR=1.189, 95%CI, 1.131 to 1.249, P<0.001), heart rate(OR=1.030, 95%CI, 1.006 to 1.055, P=0.015), SYNTAX score(OR=1.153, 95% CI, 1.089 to 1.220, P<0.001) were all independent influencing factors of Precise-DAPT score. Pearson correlation analysis showed that Precise-DAPT score was positively correlated with Gensini score(r=0.363, P<0.001) and SYNTAX score(r=0.501, P<0.001). Conclusion Precise-DAPT score is positively correlated with the severity of coronary stenosis. Patients with coronary artery disease at higher risk of bleeding have more serious coronary stenosis, and contradiction in the duration of double anti-treatment is more prominent. Therefore, the individualized treatment should be followed up closely.
