摘要
目的:分析3D/4D彩超应用于产后压力性尿失禁诊断时的价值。方法:选取2019年12月~2020年12月本院妇产科收治的50例顺产后确诊为压力性尿失禁的产妇纳入观察组,50例同期顺产健康产妇纳入对照组,通过GE Voluson E10四维彩色多普勒超声诊断仪,检测产妇的会阴、盆底等处,观察两组产妇盆底超声影像,比对平静呼吸、最大Valsalva呼吸动作状态下的膀胱颈位置等指标的具体值以及尿道内口“漏斗状”形成率和肛提肌裂孔面积大小。结果:①平静呼吸状态下,两组产妇逼尿肌厚度无显著差异(P>0.05),观察组产妇的膀胱颈位置略低于对照组,P<0.05,差异显著。在最大Valsalva动作状态下,对照组产妇膀胱尿道后角、膀胱颈移动度、尿道旋转角均优于观察组,P均<0.05。②观察组产妇尿道内口“漏斗状”形成率为64.00%,高于对照组的32.00%;肛提肌裂孔面积均值为(23.15±3.13)mm^(2),高于对照组的(16.48±2.46)mm^(2)(P均>0.05)。结论:3D/4D彩超应用于产后压力性尿失禁诊断时,准确性高。
Objective:To analyze the value of 3D/4D color Doppler ultrasound in the diagnosis of postpartum stress urinary incontinence.Methods:50 cases of postpartum women diagnosed with stress urinary incontinence were included in the observation group,and 50 cases of healthy postpartum women in the same period were included in the control group.The pelvic floor ultrasound images of the two groups were observed,and the specific values of bladder neck position under the state of calm breathing and maximum Valsalva breathing,the rate of"funnel"formation of urethral orifice and the area of levator ani hiatus were compared.Results:①In the state of calm breathing,there was no significant difference in detrusor thickness between the two groups(P>0.05),and the position of bladder neck in the observation group was slightly lower than that in the control group(P<0.05).In the maximum Valsalva movement state,the posterior angle of bladder and urethra,the mobility of bladder neck and the rotation angle of urethra in the control group were better than those in the observation group,P<0.05.②The"funnel"formation rate of the observation group was 64.00%,which was higher than 32.00%of the control group;The mean area of levator ani hiatus was(23.15±3.13)mm^(2),higher than(16.48±2.46)mm^(2)(all P>0.05).Conclusion:3D/4D color Doppler ultrasound in the diagnosis of postpartum stress urinary incontinence has high accuracy and important value.
