玻璃体腔注射康柏西普联合全视网膜光凝治疗增生性糖尿病视网膜病变的临床观察 被引量:1

Effects of intravitreal injection of conbercept combined with panretinal photocoagulation on patients with proliferative diabetic retinopathy

摘要 目的分析玻璃体腔注射康柏西普联合全视网膜光凝治疗增生性糖尿病视网膜病变(PDR)的临床效果。方法抽取2018年3月至2021年2月河南科技大学第一附属医院收治的PDR患者104例,按随机数字表法分为研究组(52例)与对照组(52例),对照组采用全视网膜光凝,研究组采用玻璃体腔注射康柏西普联合全视网膜光凝。比较两组疗效、最佳矫治视力(BCVA)、中心黄斑厚度、N1波潜伏期波幅、血清胎盘生长因子(PIGF)、基质细胞衍生因子1(SDF-1)水平和并发症发生率。结果研究组总有效率(88.46%,46/52)高于对照组(71.15%,37/52),P<0.05。术后1、3个月,研究组BCVA高于对照组,中心黄斑厚度和N1波潜伏期波幅低于对照组(P<0.05)。术后3个月,研究组血清PIGF、SDF-1水平低于对照组(P<0.05);研究组并发症发生率(3.85%,2/52)低于对照组(17.31%,9/52),P<0.05。结论玻璃体腔注射康柏西普联合全视网膜光凝治疗PDR效果显著,可抑制血管生成因子表达,减少中心黄斑厚度,改善视力水平和NI波潜伏期波幅,并可降低并发症发生率。 Objective To analyze the effects of intravitreal injection of conbercept combined with panretinal photocoagulation on patients with proliferative diabetic retinopathy(PDR).Methods A total of 104 PDR patients treated in the First Affiliated Hospital of Henan University of Science and Technology from March 2018 to February 2021 were selected and divided into study group(n=52)and control group(n=52)according to the random number table method.The control group was treated by panretinal photocoagulation,and the study group was treated by intravitreal injection of conbercept combined with panretinal photocoagulation.The curative effect,best corrected visual acuity(BCVA),central macular thickness,N1 wave latency amplitude,serum placental growth factor(PIGF),stromal cell-derived factor 1(SDF-1)levels,and the incidence of complications were compared between the two groups.Results The total effective rate of the study group was 88.46%(46/52),higher than 71.15%(37/52)of the control group(P<0.05).The BCVA of the study group was higher than that of the control group,while center macular thickness,N1 wave latency amplitude of the study group were lower than those of the control group 1 month and 3 months after operation(P<0.05).The levels of serum PIGF and SDF-1 in the study group were lower than those in the control group 3 months after operation(P<0.05).The complication rate of the study group was 3.85%(2/52),lower than 17.31%(9/52)of the control group(P<0.05).Conclusions Intravitreal injection of conbercept combined with panretinal photocoagulation has a significant effect on PDR,which can inhibit the expression of angiogenic factors,reduce the central macular thickness,improve visual acuity and N1 wave latency amplitude,and reduce the incidence of complications.
