持续与间歇蓝光治疗对新生儿高胆红素血症血清胆红素、SIL-2R、GM-CSF水平的影响

Effects of Continuous and Intermittent Blue Light Therapy on Serum Bilirubin,SIL-2R and GM-CSF Levels in Neonatal Hyperbilirubinemia

目的探讨持续与间歇蓝光治疗对新生儿高胆红素血症血清胆红素、可溶性白细胞介素2受体(SIL-2R)、粒巨噬细胞集落刺激因子(GM-CSF)水平的影响。方法选取2019年1月-2020年2月新密市妇幼保健院收治的108例高胆红素血症新生儿,按随机数字法分为对照组(n=54例)和研究组(n=54例)。两组均常规基础治疗后,对照组进行持续性蓝光照射治疗,研究组行间歇性蓝光照射,持续治疗3d后,观察比较两组患儿血清胆红素、SIL-2R、GM-CSF水平及不良反应情况。结果治疗1d、3d后研究组胆红素水平低于对照组(P<0.05)。治疗3d后研究组SIL-2R、GM-CSF水平低于对照组(P<0.05)。研究组腹泻、发热、皮疹的不良反应发生率低于对照组(P<0.05)。结论间歇蓝光治疗相对于持续蓝光照射可更好降低高胆红素血症患儿血清胆红素、SIL-2R、GM-CSF水平,治疗期间不良反应发生率更低,患儿安全性能得到保障,值得在临床中推广使用。

Objective To investigate the effects of continuous and intermittent blue light therapy on serum bilirubin,soluble interleukin-2 receptor(SIL 2 R)and granulocyte macrophage colony stimulating factor(GM-CSF)levels in neonatal hyperbilirubinemia.Methods 108 newborns with hyperbilirubinemia in Xinmi Maternal and Child Health Care Hospital from January 2019 to February 2020 were enrolled,and divided into two groups by random number table methods,each with 54 cases.Both groups received routine treatment,based on this,control group received continuous blue light therapy,while study group received intermittent blue light therapy.After continuous treatment of 3 d,serum bilirubin,SIL-2 R,GM-CSF levels and adverse reaction rate were compared between groups.Results Bilirubin levels in the study group were lower than those in the control group after one day and three days of treatment(P<0.05).The levels of SIL 2 R and GM-CSF in study group were lower than those in control group at post-treatment 3 d(P<0.05).The incidence of adverse reactions including diarrhea,fever and rash in study group was lower than that in control group(P<0.05).Conclusion Compared with continuous blue light therapy therapy,intermittent blue light therapy for newborns with hyperbilirubinemia has better effect in decreasing the serum levels of bilirubin,SIL-2 R and GM-CSF,and it also has lower adverse reaction rate and better safety,which is worthy of promotion.
