骨质疏松性椎体压缩骨折患者术后疼痛影响因素分析

Analysis of influencing factors of postoperative pain in patients with osteoporotic vertebral compression fractures

目的分析骨质疏松性椎体压缩骨折患者术后疼痛的影响因素。方法选择本院2017年9月至2020年10月收治的64例行PVP术的骨质疏松性椎体压缩骨折患者作为研究对象,按照患者术后是否存在椎间残余疼痛将其分为2组,对其性别、年龄、病因、病史、体重指数(BMI)、骨水泥渗漏、BMD及椎体高度恢复情况等相关因素进行分析。结果术后,64例患者中14例存在椎体残余疼痛(21.88%),50例无椎体残余疼痛(78.12%)。病因、病史、BMD、椎体前缘高度恢复情况、椎体中间高度恢复情况均为患者术后出现椎体残余疼痛的影响因素(P<0.05);logistic多因素回归分析结果显示:有外伤史、有既往腰伤史、BMD<-3.50g/cm^(3)、椎体前缘高度恢复<2.00mm、椎体中间高度恢复<3.00mm为患者术后存在椎体残余疼痛的危险因素(P<0.05)。结论存在外伤史或既往腰伤史、BMD值偏低、椎体前缘高度及椎体中间高度恢复效果差均为患者术后出现椎体残余疼痛的危险因素。

Objective Analysis of influencing factors of postoperative pain in patients with osteoporotic vertebral compression fractures.Methods Sixty-four patients with osteoporotic vertebral compression fractures who underwent PVP surgery,who were admitted to our hospital from September 2017 to October 2020,were selected as the research objects.The patients were divided into 2 groups according to whether there was residual intervertebral pain after surgery.Analysis of related factors such as gender,age,etiology,medical history,body mass index(BMI),bone cement leakage,BMD and vertebral height recovery.Results After surgery,14 of 64 patients had residual vertebral pain(21.88%),and 50 had no residual vertebral pain(78.12%).Etiology,medical history,BMD,anterior vertebral height recovery,and vertebral intermediate height recovery are all influencing factors for postoperative residual pain in the vertebral body(P<0.05),logistic multivariate regression analysis results show that there is a history of trauma A history of previous lumbar injury,BMD<-3.50 g/cm^(3),anterior vertebral height recovery<2.00 mm,and vertebral intermediate height recovery<3.00 mm are risk factors for postoperative residual vertebral pain(P<0.05).Conclusion History of trauma or previous lumbar injury,low BMD value,poor recovery of the height of the anterior edge of the vertebral body and the middle height of the vertebral body are all risk factors for patients with residual vertebral body pain after surgery.
