基础胰岛素起始治疗前2型糖尿病患者血糖与自我管理行为的相关性

Study on the correlation between blood glucose and self-management behavior in patients with type 2 diabetes before initiation of basal insulin therapy

目的探讨2型糖尿病患者应用基础胰岛素起始治疗前血糖与自我管理行为的相关性。方法选取2018年1月至2019年12月间浙江省仙居县人民医院收治且拟行基础胰岛素起始治疗的158例2型糖尿病患者,对所有患者人口学资料、空腹血糖(FPG)、糖化血红蛋白(HbA_(1)c)及自我管理行为情况进行调查、整理、分析。结果158例患者,FPG为(11.51±3.72)mmol/L,其中达标者共10例,占6.33%;HbA_(1)c为(9.72±2.11)%,其中达标者共15例,占9.49%;而FPG与HbA_(1)c同时达标者共3例,占1.90%,而2型糖尿病自我管理行为量表(2-DSCS)量表评分为(33.92±15.08)分,其中,自我管理行为良好者共3例,占1.90%,中等者共55例,占34.81%,差者共100例,占63.29%;经Spearman相关分析显示,2型糖尿病患者FPG及HbA_(1)c均与自我管理行为评分呈显著负相关(r_(1)=-0.399,P_(1)<0.001;r_(2)=-0.281,P_(2)=0.017)。结论2型糖尿病患者基础胰岛素起始治疗前血糖指标及自我管理行为状况均不理想,临床工作中要采取针对性措施加强患者的干预。

Objective To study the correlation between blood glucose and self-management behaviors before initial treatment with basal insulin in patients with type 2 diabetes.Methods A total of 158 patients with type 2 diabetes who were admitted to our hospital from January 2018 to December 2019 and planned to receive basic insulin initiation therapy were selected.The demographic data,fasting blood glucose(FPG),glycosylated hemoglobin(HbA_(1)c)and self-management behavior of all patients were investigated,sorted out and analyzed.Results In 158 patients,the average FPG was(11.51±3.72)mmol/L,of which 10 cases reached the standard,accounting for 6.33%.The average HbA_(1)c was(9.72±2.11)%,of which 15 cases reached the standard,accounting for 9.49%;a total of 3 cases of FPG met the HbA_(1)c standard at the same time,accounting for 1.90%,and the average score of the 2-DSCS scale was(33.92±15.08)points.Among them,there were 3 cases of good self-management behavior,accounting for 1.90%,and a total of 55 cases were moderate,accounted for 34.81%,100 cases were poor,accounting for 63.29%.Spearman correlation analysis showed that FPG and HbA_(1)c in type 2 diabetes patients were significantly negatively correlated with self-management behavior scores(r_(1)=-0.399,P_(1)<0.001;r_(2)=-0.281,P_(2)=0.017).Conclusions Before the initial treatment of basal insulin in patients with type 2 diabetes,the blood glucose index and self-management behavior are unsatisfactory.Targeted measures should be taken to strengthen patient intervention in future clinical work.
