肺癌患者放疗期间的心理弹性水平及其影响因素的研究

Study of the psychological elasticity level and influencing factors of patients with lung cancer during radiotherapy

目的研究肺癌患者放疗期间的心理弹性水平及其影响因素。方法选取2020年3月至2021年3月安徽医科大学附属阜阳医院收治的80例肺癌首次放疗患者作为研究对象,根据康纳戴维森弹性量表(CD-RIST)得分将其分为心理弹性良好组(55例,CD-RIST≥80分)及心理弹性低下组(25例,CD-RIST<80分)。比较两组患者的一般资料,分析各临床因素与心理弹性低下的相关性及影响心理弹性的独立因素。结果两组患者的应对方式、社会支持水平比较,差异有统计学差异(P<0.05)。心理弹性状态与应对方式(r=0.364,P=0.025)及社会支持水平成正相关(r=4.142,P=0.001)。logistic回归分析结果形式,应对水平消极(β=1.856,OR=3.423,95%CI=3.132~8.465)和社会支持水平低下(β=5.612,OR=3.423,95%CI=4.245~7.437)是影响心理弹性水平的独立危险因素(P<0.05)。结论心理弹性不良患者相对心理弹性良好患者应对方式更消极、社会支持水平更低下,而应对水平消极和社会支持水平低下为影响心理弹性水平的独立危险因素。

Objective To study the psychological elasticity of patients with lung cancer during radiotherapy and its influencing factors.Methods From March 2020 to March 2021,80 patients with lung cancer undergoing first radiotherapy in Fuyang Hospital Affiliated to Anhui Medical University were selected as the research subjects.They were divided into the good psychological resilience group(55 cases,CD-RIST≥80 points)and the poor psychological resilience group(25 cases,CD-RIST<80 points)based on the Connor-Davidson resilience scale(CD-RIST)scores.The general baseline data of the two groups were compared.In addition,the correlation between the clinical factors and the low mental resilience and independent factors affecting mental resilience were analyzed.Results There were statistically significant differences in coping style and social support between the two groups(P<0.05).The state of mental resilience was positively correlated with coping style(r=0.364,P=0.025)and social support level(r=4.142,P=0.001).The logistic regression analysis results suggested that negative coping level(β=1.856,OR=3.423,95%CI=3.132-8.465)and low social support level(β=2.374,OR=3.423,95%CI=4.245-7.437)were the independent risk factors of the level of mental resilience(P<0.05).Conclusion Patients with poor psychological resilience have more negative coping styles and lower levels of social support than those with good psychological resilience.Negative coping and low levels of social support are independent factors that affect the level of psychological resilience.
