多参数磁共振/经直肠超声成像认知融合靶向穿刺活检对前列腺癌的诊断价值 被引量:2

摘要 目的探讨多参数磁共振/经直肠超声(mpMRI/TRUS)成像认知融合靶向穿刺活检在诊断前列腺癌的应用价值。方法回顾性选取2017年7月至2020年12月在华南理工大学附属第六医院收治的130例疑似前列腺癌患者作为研究对象,均在mpMRI扫描后进行TRUS常规12针法系统穿刺活检和mpMRI/TRUS成像认知融合靶向穿刺活检。以常规12针法系统穿刺活检结果为金标准,观察mpMRI/TRUS成像认知融合靶向穿刺活检的诊断价值(特异度、敏感度、准确度、阳性预测值、阴性预测值)。结果mpMRI/TRUS成像认知融合靶向穿刺活检的特异度为79.22%(61/77),敏感度为79.25%(42/53),准确度为79.23%(103/130),阳性预测值为72.41%(42/58),阴性预测值为84.72%(61/72)。结论mpMRI/TRUS成像认知融合靶向穿刺活检在中高危前列腺癌中的诊断效能较高,值得临床推广应用。 Objective To explore the diagnostic value of multiparameter magnetic resonance image/transrectal ultrasound(mpMRI/TRUS)imaging cognitive fusion targeted puncture biopsy for the diagnosis of prostate cancer.Methods A total of 130 patients with suspected prostate cancer admitted to the Sixth Affiliated Hospital of South China University of Technology from July 2017 to December 2020 were selected retrospectively as the research objects,after mpMRI scanning,TRUS conventional 12-needle systematic puncture biopsy and mpMRI/TRUS imaging cognitive fusion targeted puncture biopsy were performed.The diagnostic value of mpMRI/TRUS imaging cognitive fusion targeted puncture biopsy(specificity,sensitivity,accuracy,positive predictive value,negative predictive value)was observed with the results of conventional 12-needle systematic puncture biopsy as the gold standard.Results The mpMRI/TRUS imaging cognitive fusion targeted puncture biopsy was 79.22%(61/77),sensitivity of 79.25%(42/53),accuracy of 79.23%(103/130),positive prediction of 72.41%(42/58)and negative prediction of 84.72%(61/72).Conclusion The diagnostic efficacy of mpMRI/TRUS imaging is high in medium and high risk prostate cancer and is worthy of clinical application.
