临床综合优质护理在体外膜肺氧合治疗病毒性心肌炎患者中的应用效果

Effect of clinical comprehensive high-quality nursing on patients with viral myocarditis treated by extracorporeal membrane oxygenation

目的探讨临床综合优质护理在体外膜肺氧合(ECMO)治疗病毒性心肌炎患者中的应用效果。方法选取厦门大学附属第一医院2018年8月至2020年8月收治的48例病毒性心肌炎患者作为研究对象,采用随机数字表法分为综合组与对照组,每组各24例。两组患者均予以ECMO治疗,综合组患者予以综合优质护理,对照组患者予以常规护理。比较两组患者护理2月后心肌酶谱指标水平与心功能指标水平差异。结果综合组和对照组护理前心肌酶谱与心功能指标水平比较,差异无统计学意义(P>0.05);护理2月后,综合组各项心肌酶谱指标水平低于对照组,各项心功能指标水平高于对照组,差异有统计学意义(P<0.05)。两组患者护理2月后,各项心肌酶谱指标水平低于护理前,心功能指标水平高于护理前,差异有统计学意义(P<0.05)。结论综合优质护理用于行ECMO治疗的病毒性心肌炎患者的临床效果显著,可明显改善患者心功能,建议应用推广。

Objective To explore the application effect of clinical comprehensive high-quality nursing in the treatment of patients with viral myocarditis by extracorporeal membrane oxygenation(ECMO).Methods A total of 48 patients with viral myocarditis admitted to the First Affiliated Hospital of Xiamen University from August 2018 to August 2020 were selected as clinical research objects,they were divided into comprehensive group and control group according to random number table method,with 24 cases in each group.Patients in the two groups were treated with ECMO,patients in the comprehensive group were given comprehensive high-quality care,and patients in the control group were given routine care.The differences in the levels of myocardial enzyme spectrum and cardiac function indexes were compared between the two groups after 2 months of nursing.Results There was no statistical significance in the levels of myocardial enzymes and cardiac function indexes between the two groups before nursing(P>0.05).After 2 months of nursing,the levels of myocardial enzymes in the comprehensive group were lower than those in the control group,the level of cardiac function in the comprehensive group was higher than that in the control group,the differences were statistically significant(P<0.05).After 2 months of nursing,the levels of myocardial enzymes in the two groups were lower than those before nursing,and the levels of cardiac function indexes in the two groups were higher than those before nursing,the differences were statistically significant(P<0.05).Conclusion Comprehensive high-quality care for patients with viral myocarditis undergoing ECMO treatment has significant clinical effects,which can significantly improve the heart function of patients,and it is recommended to be applied and promoted.
