肿瘤药学门诊规范化建设实践与成效

Practice and efficiency of standardized construction of the oncology pharmacy outpatient service

目的:探讨肿瘤药学门诊的工作模式并分析其运行效果,为药师开设药学门诊提供参考。方法:通过调查肿瘤患者药学服务需求,参考美国药物治疗管理模式服务要点,结合门诊实际工作流程,药师设计了开发药师工作站,建立了规范化工作流程并对肿瘤药学门诊的运行效果进行初步评价。结果:2018-06-06至2020-12-31,上海交通大学附属第六人民医院肿瘤药学门诊共有323例患者就诊951例次,平均每天约7.4例次。共发现并处理97例次药物相关问题,已解决完成88例次药物相关问题,解决完成率为90.7%。药师记录并处理患者药品不良反应244例次,其中97.5%的药品不良反应经药师干预后改善。另外,药师处理患者其他伴发疾病619例次。结论:药师通过药学门诊参与患者的药物治疗管理,有利于保障患者用药安全。

Objective:To explore the working mode of the oncology pharmacy outpatient service and evaluate its operational efficiency,so as to provide reference for pharmacists to offer better clinical pharmaceutical service.Methods:Through investigation on the pharmaceutical care needs of cancer patients,with reference to the key elements concerning drug therapy management and in combination of practical clinical working flow,the pharmacist work station was designed and developed successfully.At the same time,standardized working procedures were established,and the operational efficiency of oncology pharmacy outpatient service was evaluated preliminarily.Results:From June 6,2018 to December 31,2020,there were totally 323 outpatients visits(951 case times)in the oncology pharmacy outpatient service of the hospital,with an average of 7.4 case times per day.Within this period,we encountered and processed 97 case times of drug-related problems,of which 88 were sucessfully solved and completed,with a completion rate of 90.7%.Totally,the pharmacists recorded and handled 244 case times of adverse drug reactions,of which 97.5%case times were improved with pharmacist intervention.In addition,clinical pharmacists handled another 619 case times of accompanied diseases.Conclusion:Pharmacist involvement in medication management of the patients through pharmacy outpatient service could help to ensure medication safety.
