湖南省公立医院经济运行分析

Analysis on the economic operation in Hunan public hospital

[目的]了解湖南省公立医院2014-2018年经济运行状况,为湖南省公立医院改革提供政策建议和参考。[方法]通过卫生统计年报、卫生财务年报等资料收集相关数据,采用描述性统计和对比分析方法进行分析。[结果]2014-2018年,湖南省公立医院医疗服务量稳步增长,医师人均每日负担小幅下降,病床使用率也呈下降趋势,但病床使用率仍高于基层医疗机构;总收入逐年上涨,药品收入占比明显下降,财政补助收入占比偏低;总支出逐年增加,药品支出规模下降,从2014年的40.6%下降到2018年的33.5%,人员支出和卫生材料支出增加;收支结余和收支结余率持续下降,年末未弥补亏损值越来越大。[结论]湖南省公立医院的财政补助收入占总收入比重偏低;“以药补医”现象得以缓解,医务人员劳动价值得以体现;公立医院业务量稳定增长,卫生材料费和人员支出占比增加。

Objective To know the economic operation status of Hunan public hospitals from 2014 to 2018,in order to provide policy suggestions and reference for the reform of public hospitals in Hunan Province.Methods It collected relevant data from health statistics yearbook and health financial annual report,descriptive statistics and comparative analysis method were used to analyze the data.Results From 2014 to 2018,the quantity of medical services in public hospitals was steadily increased,per capita daily burden of doctors was decreased within a narrow range,bed utilization ratio also showed a downward trend but it was still higher than primary medical institutions;The total income increased year by year,the proportion of drug income decreased significantly,and the proportion of financial subsidy income was low;The total expenditure increased year by year,the scale of drug expenditure decreased from 40.6%in 2014 to 33.5%in 2018,and the expenditure on personnel and health materials increased;The balance of revenue and expenditure and the rate of balance of revenue and expenditure continued to decline,and the value of undistributed losses at the end of the year was bigger and bigger.Conclusions The proportion of financial subsidy income in the total income of public hospitals is low;The phenomenon of“supplementing medicine service with drug benefit”has been alleviated,and the labor value of medical staff has been reflected;The portfolio of public hospitals'services has steadily increased,and the proportion of health materials expenditure and personnel expenditure have increased.
