摘要
目的:分析无创通气(NPPV)和慢性阻塞性肺疾病(COPD)急性加重次数和持续时间的关系。方法:2019年1-12月收治COPD患者150例,随机数字表法分为两组,各75例。观察组采用NPPV联合常规治疗;对照组采用常规治疗。比较两组患者肺功能及气血指标水平;分析COPD急性加重次数和持续时间与NPPV的相关性。结果:观察组肺活量(FVC)、第1秒呼气容积(FEV1)、峰流速(PEF)及最大呼气中期流速(MMEF)等肺功能指标水平均高于对照组,差异有统计学意义(P<0.05)。观察组治疗后6 min步行距离高于对照组,差异有统计学意义(P<0.05)。观察组治疗后动脉血氧分压水平高于对照组,动脉二氧化碳分压水平低于对照组,差异有统计学意义(P<0.05)。NPPV与COPD加重次数和持续时间存在明显相关性,且均为负相关(P<0.05)。结论:NPPV可有效降低COPD急性加重次数和持续时间,对患者具有积极影响。
Objective:To analyze the relationship between noninvasive ventilation(NPPV)and the number and duration of acute exacerbation of COPD.Methods:From January to December 2019,150 patients with COPD were selected.They were randomly divided into two groups with 75 cases in each group.The observation group was treated with NPPV combined with routine treatment.The control group was treated with routine treatment.We compared the levels of pulmonary function and blood gas between the two groups.The correlation between the number and duration of acute exacerbation of COPD and NPPV was analyzed.Results:In the observation group,the lung function indexes such as FVC,FEV1,PEF and MMEF were higher than those in the control group,the difference was statistically significant(P<0.05).The walking distance 6 minutes after treatment in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).In the observation group,after treatment,the PaO2 level was higher than that in the control group,and the PaCO2 level was lower than that in the control group,the difference was statistically significant(P<0.05).PNNV was significantly correlated with the number and duration of exacerbation of COPD,and they were all negatively correlated(P<0.05).Conclusion:NPPV can effectively reduce the number and duration of acute exacerbation of COPD,and has a positive impact on patients.
