前交叉韧带重建患者步态对称性变化及其与步速相关性研究

Study of Gait Symmetry Changes in Patients with Anterior Cruciate Ligament Reconstruction and Correlation with Gait Speed

目的:探究前交叉韧带(anterior cruciate ligament,ACL)重建术后患者步行时的步态对称性变化及与步速的相关性.方法:选取单侧ACL重建术后患者(观察组)以及步态正常的健康人群(对照组)各40例,每组受试者分别进行步态测试,计算和收集受试者的步速、单支撑相时间、双支撑相时间、COP-Y及步态对称性参数.结果:观察组步速较对照组降低,差异有统计学意义;观察组患侧单支撑相、COP-Y与健侧相比降低,差异有统计学意义;观察组双支撑相时长与对照组相比延长,差异有统计学意义;观察组与对照组相比,观察组单支撑相时间、COP-Y对称指数,与对照组相比差异有统计学意义.观察组步速与单支撑相时间对称性指数成高度负相关(r=-0.87,P<0.01),与COP-Y对称性指数成高度负相关(r=-0.74,P<0.01).结论:前交叉韧带重建患者步态呈不对称性变化,步速、患侧单支撑相时间降低,双支撑相时长和COP-Y增加,患侧单支撑相时间对称性指数和COP-Y对称性指数与步速存在负相关性.

Objective:To explore the symmetry changes of patients"gait during walking after anterior cruciate ligament(ACL)reconstruction and its correlation with gait speed.Methods:Selected patients after unilateral ACL reconstruction(experimental group)and healthy people with normal gait(control group)40 cases each.Subjects in each group were tested at natural pace,and the subjects were extracted and counted.The pace,single support phase time,dual support phase time,and the displacement of the center of pressures(COP)on the sagittal axis(COP-Y).Results:The pace of the experimental group was lower than that of the control group,and the difference was statistically significant;the experimental group had single support phase,COP-Y compared with the uninfected side,the difference is statistically significant;the duration of dual support phase in the experimental group and the control group.Compared with the prolongation,the difference is statistically significant;the experimental group is compared with the control group,the single support phase time and COP-Y symmetry index of the experimental group are compared with those of the control group Compared to increase,the difference is statistically significant.The difference is statistically significant;the experimental groups pace is highly negatively correlated with the single-support phase time symmetry index(r=-0.87,P<0.01),and highly negatively correlated with the COP-Y symmetry index(r=-0.74,P<0.01).Conclusion:The gait parameters of patients after ANTERIOR cruciate ligament reconstruction showed an asymmetric pattern,which showed that the walking speed,the single support phase time of the affected side decreased,and the COP-Y increased.The time symmetry index and COP-Y symmetry index of single support phase on the affected side were negatively correlated with walking speed.
