临床药师参与自控硬膜外麻醉分娩镇痛管理的实践与分析

Practice and analysis of clinical pharmacists participating in the management of patient-controlled epidural anesthesia labor analgesia

目的探究临床药师参与自控硬膜外麻醉(PCEA)分娩镇痛管理的必要性及意义。方法临床药师参与PCEA分娩镇痛管理,包括产前的科普、宣教和答疑,产后的药学监护、疼痛评估及镇痛满意度调研。对产妇的基本信息、PCEA分娩镇痛异常情况、分娩镇痛前后疼痛评估、镇痛满意度评分以及母儿结局进行分析。结果 2019年6月至2020年6月,临床药师共参与了391例产妇的分娩镇痛管理。其中,23.79%的产妇产后诉穿刺点酸疼,18.41%的产妇出现产时发热,63.68%的产妇对PCEA分娩镇痛效果表示非常满意,母儿结局良好。结论采用PCEA分娩镇痛的产妇存在硬膜外相关产时发热等异常情况,临床药师有必要参与PCEA分娩镇痛的管理,为产妇提供精准的药学监护。

AIM To explore the necessity and significance of clinical pharmacists participating in the management of labor analgesia with patient-controlled epidural anesthesia(PCEA). METHODS The clinical pharmacists participated in PCEA labor analgesia management, including prenatal science popularization, education and answering questions,postpartum pharmaceutical care, pain evaluation and analgesic satisfaction survey. The basic information of parturient women, abnormal conditions of PCEA labor analgesia, pain evaluation before and after labor analgesia, analgesic satisfaction score and maternal and infant outcomes were analyzed. RESULTS From June 2019 to June 2020, the clinical pharmacists participated in the labor analgesia management of 391 pregnant women. Among them, 23.79% of the parturients complained of pain at the puncture point, 18.41% of the parturients had fever during delivery, 63.68% of the parturients were very satisfied with the analgesic effect of PCEA, and the outcome of the mother and child was good.CONCLUSION The parturients with PCEA delivery analgesia may have abnormal conditions such as epidural related intrapartum fever. It is necessary for clinical pharmacists to participate in the management of PCEA delivery analgesia and provide accurate pharmaceutical care for parturients.
